Job summary
From
2028, it is deemed that 55,000 more people will survive five years or more
following a diagnosis of cancer. Currently this figure is approximately 50%
within our local area and the role of the Cancer Care Co-ordinator is to work collaboratively
with the GP Practices within our Primary Care Network (PCN) in order to help increase
this figure up to 75%.
The Cancer Care Co-ordinator is a developing post and will have responsibility
for supporting and developing the co-ordination and management of the Early Detection
& Prevention of Cancer scheme across the Southport and Formby PCN and ensuring that the early part of the patients cancer journey
is as seamless as possible. The Co-ordinator will understand the health
inequalities within the Primary Care Network and its impact on those at every
stage of the cancer pathway. The
Co-ordinators role will work within the wider network of support wrapped around
the patient and their GP practice.
The Cancer Co-ordinator will work closely with the Cancer Lead GP to
agree targeted workplans and will be required to work autonomously and on their
own initiative in order to deliver these workplans, whilst being supported by
the PCN Operations Manager.
The role is advertised as being full-time however part-time applications will be considered.

assist the Primary Care Network (PCN) in delivering improvements to the
services we provide our patients in relation to patient participation in
national cancer screening programmes.

support of collaborative working the post holder will form productive and
supportive relationships with practice and PCN staff who have the skills,
knowledge, and remit to contribute to this work. e.g., GP practice nonclinical
cancer champions, social prescribers, pharmacists, medical secretaries etc
The post holder
will assist the PCN member practices to evaluate their screening uptake and
work to engage with health inclusion groups (i.e. our most marginalised
patients and those underrepresented) and to reduce health inequalities. This
will include working alongside practices to enhance processes to track and
follow-up screening of non-responders, calling patients and identifying themes
to understand reasons for non-attendance/engagement.

Provide
advice and support to practices on cancer audit/referral review of cancer
diagnoses. To work with practices to collate the learning from case reviews to
identify any trends or learning.

Health Inclusion Strategy.

with support (not delivery) to host peer-to-peer learning events that look at
data and trends in diagnosis and screening across the network. Including
appropriate contributors from other organisations.

Source
appropriate resources, training, and system examples from appropriate
organisations such as the Cancer Alliance, Cancer Research UK, Macmillan, and
other local authority partners.

Be a point of
contact for PCN member practices to assist them in developing and implementing
their cancer screening improvement action plans

Build
relationships with community assets and develop a service directory of support
for screening and pre-diagnosis for the patients within our PCN, and a Library
of PCN data packs and other resources to support the delivery of information
available to patients in a wide variety of formats to meet the needs of all
patient groups, including those with learning disabilities, and those people
for whom English is not a first language.

Review practice
coding for report building and templates to ensure consistency across the PCN
and accuracy of data. Identify coding anomalies and liaise with Ardens
(template and reports used by all member practices).
