Job summary
Southport and Formby Health (SAFH) are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.
Southport and Formby Health work collaboratively with Southport and Formby Primary Care Network (PCN). Southport and Formby PCN is a group of 15 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and health care for our local population of around 129,000 people.
We are looking to recruit a creative and motivated individual for the role of Digital Transformation Care Coordinator. The role will support the digital hub at Southport and Formby Primary Care Network (PCN).
The role will be integral to the digital transformation of our organisation and offers exciting opportunities in digital healthcare and innovation. The individual will work closely with senior management and the specialised care teams within the Primary Care Network to develop digital health services.
The role is advertised as being full-time however part-time applications will be considered.

Main duties of the job
The role will support the digital hub at Southport and Formby Primary Care Network (PCN). The post will help to implement several core functions:
- helping the PCN and its member practices understand and improve general practice appointments data
- provide digital health support to the Southport and Formby population, specifically related to mental health, diabetes, and weight management
- assist member practices in digitising workflows
- and to assist the PCN to implement new services relating to collaborative working to reduce pressure in general practice within digital workstreams.
