Job summary

Southport and Formby Health (SAFH) are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.

Southport and Formby Health work collaboratively with Southport and Formby Primary Care Network (PCN). Southport and Formby PCN is a group of 15 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and health care for our local population of around 129,000 people.

We are looking to recruit a creative and motivated individual for the role of Digital Transformation Care Coordinator. The role will support the digital hub at Southport and Formby Primary Care Network (PCN).

The role will be integral to the digital transformation of our organisation and offers exciting opportunities in digital healthcare and innovation. The individual will work closely with senior management and the specialised care teams within the Primary Care Network to develop digital health services.

The role is advertised as being full-time however part-time applications will be considered.

Main duties of the job

The role will support the digital hub at Southport and Formby Primary Care Network (PCN). The post will help to implement several core functions:

  • helping the PCN and its member practices understand and improve general practice appointments data
  • provide digital health support to the Southport and Formby population, specifically related to mental health, diabetes, and weight management
  • assist member practices in digitising workflows
  • and to assist the PCN to implement new services relating to collaborative working to reduce pressure in general practice within digital workstreams.

Job Responsibilities include but are not limited to;

EMIS and Digital Systems Support:

- Create and run searches using EMIS to extract and analyse relevant data.

- Develop and maintain templates, protocols, and concepts in EMIS to streamline practice workflows.

- Utilise digital clinical tools such as Ardens, iPlato, and Accurx to enhance digital healthcare delivery across practices.

Collaboration and Communication:

- Collaborate with PCN staff, such as GP Digital Champions, practice IT leads, Practice Managers, Social Prescribers, and Pharmacists, to drive digital transformation and service improvements.

Patient Support and Health Inequality Reduction:

- Assist PCN practices in engaging hard-to-reach populations and reducing health inequalities, with a focus on veterans, carers, pre-diabetics, and those requiring weight management support.

- Use population health intelligence tools to proactively identify patients requiring targeted interventions and support.

- Help patients access self-management education courses, peer support, and other interventions to empower them to take control of their health and well-being.

Care Navigation and Coordination:

- Provide care coordination for patients and their carers, working closely with Social Prescribing Link Workers, Health and Wellbeing Coaches, and other primary care professionals.

- Assist patients in accessing services related to mental health, diabetes, weight management, and other lifestyle support, ensuring they are linked to the right care pathways.

Drive Service Efficiency:

- Drive integrated working across the PCN to enhance collaboration between practices.

- Promote efficiency and reduce duplication by standardising processes and workflows.

- Support the reduction of variation between practices, ensuring consistent service delivery across the network.

Project and Task Management:

- Manage work and communication using MS Teams to ensure effective coordination and task tracking across projects.

- Monitor and report on digital transformation progress: Track the implementation and impact of digital initiatives across the PCN and provide regular reports to management on improvements and areas for further development.
