Job summary

We are now in an exciting new phase of development, taking our services closer and into peoples homes.

Based in the centre of Poole, we run satellite services in Swanage, Wareham, and Wimborne. We also offer respite and social isolation sits for our patients and carers, run a digital virtual service, Family Support and Bereavement service, Lymphoedema clinics for cancer patients, Breathlessness clinics, a Clinical Complementary Therapy service and an Occupational Therapy service.

Our HCAs must work flexibly, be willing to travel and work as part of a multidisciplinary team, playing a key role in offering high quality care to all our patients. Reporting to the Senior Nurse, HCAs are an integral part of the efficiency and effectiveness of Day Hospice Services, contributing to maintaining our high standards of palliative care.

We are looking for health care professionals, with significant experience of providing high quality care, ideally with an interest in or experience in palliative care.

If you have a passion for providing high quality care, have good interpersonal and communication skills Are IT competent and have experience in all of the above we would love to hear from you This role requires you to travel to our satellite provisions across Poole and the Purbecks, and undertake patient home visits. Main duties of the job

  1. To act as an assistant in the delivery of high-quality holistic care to patients, including
  2. Observing and reporting any significant changes in a patients physical or psychological condition care given and document in care plan
  3. Assist with patient personal care including bathing and toileting, urinary catheters, ileostomy, and colostomy care.
  4. Work closely and as directed by the RN in the delivery of home visits to include personal care respite sits and carers support
  5. Be involved and encourage patients in activities such as art and gentle exercises
  6. Bringing and using extended skills such as, hand massage and manicures
  7. Deliver nursing care as delegated by the RN, including dressing of minor wounds.
  8. Provide emotional support allowing time for patients and family/carers to talk, using good communication skills.
  9. Assist with the preparation and serving of meals and feeding of patients where required. Encourage adequate diet as advised by the registered nurse. To assist patients with meal selection as appropriate to any special dietary requirements
  10. Report immediately to the RN on duty any complaints, comments, or requests for information by patients or their visitors
  11. Assist in the routine care, cleaning and tidying of nursing equipment.
  12. Contribute to maintaining and developing the Day Hospice as an environment conducive to providing high standards of palliative care in all our centres.

We are a committed team, who work hard to provide the highest quality of care and support to people living with life limiting illness across Poole, Purbeck, and East Dorset.

We are based in Poole, with satellites in Swanage, Wimborne, Bournemouth and Wareham. We currently offer Day Hospice, Virtual Day Hospice, Lymphoedema Services for cancer patients, Bereavement and Family Support Services, Complimentary Therapy, Breathlessness Services and are developing a hospice at home service.

2.9. Ensure patient area environment and equipment is clean, tidy and in working order.

3. Assist with the preparation and serving of meals and feeding of patients where required. Encourage adequate diet as advised by the registered nurse. To assist patients with meal selection as appropriate to any special dietary requirements

4. Report immediately to the RN on duty any complaints, comments, or requests for information by patients or their visitors

4.1. Answer the telephone, take messages, and refer to the RN on duty for instructions.

4.2. Assist with errands, as requested by the RN on duty.

4.3. Assist in the routine care, cleaning and tidying of nursing equipment.

4.4. Develop knowledge and skills through education by:

4.5. Attending all mandatory training to include BlueStream Academy online training provided by LMT. Complete our extensive in-house competency books

4.6. Attending appropriate in-house and external education sessions, conferences, and seminars.

4.7. Continue to develop your skills and understanding of palliative care Attending appropriate courses.

5. Participating in individual performance review and appraisal giving and receiving feedback about Clinical practice and development needs

6. Follow infection control procedures/policies as appropriate.

7. Attend 6 weekly 1:1s with line manager.

8. Work as a member of the nursing team with supervision from a registered nurse and to contribute to maintaining and developing the Day Hospice as an environment conducive to providing high standards of palliative care in all our centres.

9. Be responsible for providing support to the registered nurses in the provision of care given to patients and their families.

10. The post-holder has no direct budget responsibility but must be aware of cost implications and make effective use of resources.
