Job summary
The
Patient Care Administrator will work with the GPs and other primary care
professionals within the PCN to identify a caseload of patients to access
medical services to improve their health. The primary focus of the role will be
supporting individuals registered across the practices with Learning
Difficulties (LD) and serious mental illness (SMI).
The Patient Care Administrator will support
patients and help them to manage their needs and to access support. They will provide coordination and navigation
for people and their carers across health and care services, working closely
with the Community Link Advisers (Social Prescribers) and other primary care
colleagues.
A key responsibility
within this role will be to support early cancer diagnosis and give appropriate
support. Use of C the Signs to record data. Work with appropriate outside
agencies to increase uptake of non-responders.
