We are seeking a highly motivated Registered Nurse to work within a wider team of nurses and health care assistants. You will work under the direction of a District Nurse, to assess, implement and evaluate person centred care for a caseload of patients, undertaking venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control. On achieving all relevant competencies, the post holder will be reviewed for a Band 6 Senior Community Nurse post.

Post holders will be available to work Monday to Sunday between 8am and 8pm.A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Main duties of the job

Job Purpose:o The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life.o The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile working.o The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control and on successfully attaining all the competencies required will be reviewed for a Band 6 Senior Community Nurse post.

  • Keeps up to date with evidence-based practice and research related to nursing and health care in the community
  • Supervisor / Mentorship qualification (within the first 2 years of post)
  • Evidence of continuing professional development
  • Ability to assess nursing needs and implement nursing care and treatment

Desirable

  • Evidence of professional skills relevant to Community Nursing i.e. venepuncture, male catheterisation (or willingness to undertake within 6 months of post)
