Job summary

For the Admiral Nurse post we are looking for a Registered Nurse (adult/mental health/learning disability) with a passion for, and demonstrable track record of, providing person-centred and relationship-centred dementia care with a sense of advocacy and equalities for this client group. The post holder will need to have the ability to work in a variety of settings, across South Tyneside combining direct work with families with supporting the best practice of health and social care professionals to improve the experience and quality of life of people and families affected by dementia. Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities. Youll become a welcome part of a network of Admiral Nurses and will have the opportunity to access relevant post-graduate training and develop professionally, informed by the Admiral Nurse Competency Framework.

Main duties of the job

The Admiral Nurse will support, deliver and coordinate relationship-centred dementia care within the operational areas of South Tyneside Health Collaboration, in order to improve the experience of individuals / carers/families affected by dementia with complex needs. In line with the Admiral Nurse Competency framework at a specialist level, the post holder will provide: Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs Facilitation of education and training for staff in the delivery of evidence-based dementia care Support with policy development and quality improvements in relation to dementia care Consultancy to other health and social care professionals regarding dementia

South Tyneside Health Collaboration (STHC) is working with Dementia UK and South Tyneside ICB in order to develop an integrated Dementia Wellbeing Pathway to ensure consistent and increasing levels of expertise are available, affording wrap-around support for people living with dementia and their families to maintain and enhance peoples quality of life. This initiative has enabled recruitment of an admiral nurse team to support our aim.

Essential

  • Registered Nurse (RNM/RNMH/RGN/RNLD)
  • Post-graduate training/ education in dementia/older people
  • Willingness to complete Dementia UKs Pre Admiral Nursing e-learning course prior to starting in post
  • Willingness to work towards continued professional and practice development through Dementia UKs Admiral Nurse Academy, including the level 6 or 7 Admiral Nurse Competency Framework Module.

Desirable

  • Additional nursing qualification
  • Degree level qualification in dementia / older peoples care
  • Mentorship Qualification
  • Relevant teaching qualification

Experience

Essential

  • Post-registration clinical experience of working with people with dementia and their carers/supporters
  • Experience of completing holistic nursing assessments
  • Experience of providing bio-psychosocial interventions
  • Experience of facilitating and delivering training
  • Experience of working as an autonomous practitioner

Desirable

  • Experience of managing care across care transitions
  • Experience of working with patients/ clients to develop practice
  • Experience of recent involvement in practice/ service development
