Job summary

STHC will employ Admiral Nurses, and they will be located where health and social care teams have maximised integration, of which the Admiral Nurses will be an integral part. There will be close links with the memory protection service and the older persons mental health team. Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to both people with dementia and their families / informal carers. Admiral nurses in South Tyneside will become part of a rapidly expanding team of professionals based within primary care networks working together with GP practices to deliver holistic care. The Admiral Nursing team will be based within multi-disciplinary/agency community teams and work alongside nurses, therapists, primary care, social care and third sector providers, amongst others, you will also benefit from being a part of a Clinical Network of Admiral Nurses working across North East England and beyond.

Main duties of the job

See attached supporting documentation: For the Admiral Nurse Team Manager / Clinical Lead post we are looking for an exceptionally skilled Registered Nurse (adult/mental health/learning disability) with a passion for, and significant demonstrable track record of, providing person-centred and relationship-centred dementia care with a strong sense of advocacy and equalities for this client group. Combining direct work with families with supporting the best practice of health and social care professionals, you will call upon your impressive knowledge of bio-psychosocial interventions to improve the experience and quality of life of people and families affected by dementia. The post holder will need to have the ability to work in a variety of settings, across South Tyneside. The role will include seeing individuals, carers and families individually, managing/supervising a team of two, running groups and providing education/ training, as well as balancing a proxy-clinical caseload - all whilst setting-up, developing, maintaining and evaluating a new service.

  • Additional nursing qualification. Masters level qualification in dementia / older peoples care. Mentorship Qualification. Relevant teaching qualification
