Job summary
For the Band 6 Admiral Nurse post we are looking for a Registered Nurse (adult/mental health/learning disability) with a passion for, and demonstrable track record of, providing person-centred and relationship-centred dementia care with a sense of advocacy and equalities for this client group. The post holder will need to have the ability to work in a variety of settings, across South Tyneside combining direct work with families with supporting the best practice of health and social care professionals to improve the experience and quality of life of people and families affected by dementia. Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities. You’ll become a welcome part of a network of Admiral Nurses and will have the opportunity to access relevant post-graduate training and develop professionally, informed by the Admiral Nurse Competency Framework.
Main duties of the job
The Admiral Nurse will support, deliver and coordinate relationship-centred dementia care within the operational areas of South Tyneside Health Collaboration, in order to improve the experience of individuals / carers/families affected by dementia with complex needs. In line with the Admiral Nurse Competency framework at a specialist level, the post holder will provide: • Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs • Facilitation of education and training for staff in the delivery of evidence-based dementia care • Support with policy development and quality improvements in relation to dementia care • Consultancy to other health and social care professionals regarding dementia

South Tyneside Health Collaboration provides services to improve the health and wellbeing of patients across South Tyneside. This includes supporting the three Primary Care Networks across 21 practices and a range of other patient-centred services, working closely with healthcare providers to deliver the best possible care. South Tyneside Health Collaboration (STHC) is working with Dementia UK and South Tyneside CCG in order to develop an integrated Dementia Wellbeing Pathway to ensure consistent and increasing levels of expertise are available, affording ‘wrap-around’ support for people living with dementia and their families to maintain and enhance people’s quality of life. This initiative has enabled recruitment of an admiral nurse team to support our aim.
