Job summary
Join our dynamic team at the Havering Liberty Primary Care Network as an Enhanced PCN Nurse (Frailty). Based at the St Georges Health and Wellbeing Hub in Hornchurch, you will cover all Havering Liberty Primary Care Practices. You will work closely with other healthcare professionals and key partners to support older individuals living with frailty or complex needs, helping them live longer and healthier lives.
Main duties of the job
As an Enhanced PCN Nurse, you will be responsible for undertaking comprehensive geriatric health assessments, coordinating care planning, and collaborating with various services and professionals. You will provide clinical leadership, act as a role model, and ensure high-quality care through evidence-based practice. This role requires a Registered Nurse with experience in primary care, frailty, or community elderly care, and the ability to work effectively within a multi-disciplinary team. We are a collaborative healthcare network of five General Practices serving the population of Upminster and Hornchurch with an extended multi-disciplinary team seeking to provide accessible, patient-centred, and high-quality primary care services that promotes the health and well-being of our community and our workforce.
Havering Liberty PCN is led by our Clinical Director and PCN Leadership Team who work together with the GP leads and their practice colleagues. The PCN is supported by its GP Federation, Havering Health Ltd, who are the employing body for the workforce of the network. The GP Federation supports the provision of innovative, high-quality and responsible health care services for Haverings residents

The post holder will use their assessment and diagnostic skills to assess and treat patients. The post holder will be an experienced nurse, collaborating with colleagues from primary care, community and secondary care, as appropriate. The post holder will be based at a primary care site and will routinely visit patients in their own homes; they may at times be expected to work in other settings, e.g. clinic setting.
The post holder will:
- be a Registered Nurse with experience of working in a relevant field, e.g. primary care, frailty or community elderly care
- alongside other colleagues, support the team and delegate work to the care coordinators and social prescriber as appropriate
