Job summary

An exciting opportunity has arisen here at Bromleag Care Practice for an Advanced Nurse Practitioner to join our growing multi-disciplinary team. This post holder will work as one of a team of GPs, Nurses, Pharmacists and more, delivering general practice services to patients at Bromleag Care Practice. We provide comprehensive primary care to patients residing in Care Homes and Extra Care Housing within the Borough of Bromley. At all times the post holder must act in a professional manner consistent with the code of conduct expected from medical practitioners representing Bromleag Care Practice and the NHS.

Main duties of the job

-Conduct comprehensive clinical assessments and provide expert care to patients

-Diagnose and manage both acute and chronic conditions independently

-Prescribe medications within scope of practice and in line with local guidelines

-Contribute to health promotion, disease prevention and long-term condition management

-Complete Duty, on the day, phone calls from 8am-12am (timings may vary dependant on the start time of shift), completing duty phone calls in their entirety, (including completing management plans, prescribing medication and making referrals)

-Routine and emergency home visits for patient reviews and queries for the remainder of the shift. Ensuring appropriate emergency GP visits to the homes

-The enhanced service will utilise a multi-disciplinary team approach to proactive care, integrated case management and urgent care during core general practice hours of 8am to 6:30pm Monday till Friday

-Provide accessible care: providing a personalised, responsive, timely, flexible and accessible service

-Making professional, autonomous decisions in relation to presenting problems, whether self -referred or referred from other health care workers within the organisation

-Provide clinical care using expert knowledge and developed clinical skills in assessing, implementing, and monitoring patients to ensure that the quality of patient care is maintained at the highest standard

  • -Completed MSc or PGDip in Advanced Clinical Practice (or equivalent recognised ANP qualification)
  • -Independent Non-Medical Prescriber (V300)

  • -Knowledge of QOF
