Job summary

The Digital & Transformation Lead will work with the clinical and operational team within Orpington PCN to identify opportunities to make tangible positive transformation, and provide the inspiration, resource and tools to execute changes in a collaborative way.

As Digital & Transformation Lead, you would be expected to support increased access to care for patients, by supporting the adoption and/or optimisation of new technology and other initiatives to improve the care offer and enabling practice staff to work more effectively to support the sustainability of general practice services.

Main duties of the job

Work closely with management, Clinical Directors, GP Partners, Practice staff to understand current service models and admin processes identify challenges, and agree priories for improvement and then support delivery. Work closely with the ICB Primary Care/Digital teams to ensure local transformation plans and activity are aligned to the ICB priority e.g. improving access, PCN Development, the ICB Digital Strategy and recommendations with the Fuller Report.

Support the development and maturity of the PCN ensuring that process and policies are fully aligned, standardised and maintained

Support Population Health Management

Support PCN DES requirements

Support the delivery of ongoing improvement to administrative and clinical services drawing on your experience in business/service improvement with expertise in transformational change and understanding of general practice systems and process

Develop and manage an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering potential impacts on the wider organisation, and determining resource requirements

Provide feedback to the executive management team on progress of current initiatives, capacity for change, and issues so that plans can be adapted in response

Use data, improve data quality, to understand demand, capacity and activity and drive improvements in patient experience of access, operational efficiency including better matching capacity to need and staff experience at work
