Job summary

The individual will work alongside the multi-disciplinary practice team to effectively plan and implement a high quality Health and Wellbeing Coaching service as part of the broader primary care team. The post holder will work autonomously and support patients of all ages and their carers to manage their physical and mental health and wellbeing, build community resilience and make informed decisions and choices when their health changes. The individual will deliver a universal and proactive offer of support to people with long-term physical and mental health conditions to build knowledge, skills and confidence to live well with their health condition. The will also deliver intensive and integrated approaches to empower people with more complex needs, including those living with multi-morbidity, to experience coordinated care and support that supports them to live well, helps reduce the risk of becoming frail, and minimizes the burden on treatment.

Main duties of the job

This role requires the individual to be able to assess patient needs, helping patients to gain and use knowledge, skills and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals. The post holder will need to communicate effectively with a wide range of multi-disciplinary colleagues both in house and externally and be able to take the complex needs of individuals and the varying requirements of the post into account in order to prioritise work undertaken. The post holder will actively work to continually develop the service in a patient centered, evidence based context.

Bromley GP Alliance (BGPA) is a network of 39 Bromley Practices which are working collaboratively to enhance the health and wellbeing of Bromley residents, covering over 310,000 patients, nearly 95% of the Bromley population

Responsibilities:

  • Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.

  • Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in manging their own health and wellbeing, to live independently, and improve their health outcomes through:

    • providing interventions such as self-management education and peer support; and

    • supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual; and

    • working with the social prescribing service to connect them to community-based activities which support their health and wellbeing.

  • Provide support to local community groups and work with other health, social care and voluntary sector providers to support the patients health and well-being holistically.

  • Ensure that fellow PCN staff are made aware of health coaching and social prescribing services and support colleagues to improve their skills and understanding of personalised care, behavioural approaches, and ensuring consistency in the follow up of peoples goals where an MDT is involved.

  • Raise awareness within the PCN of shared decision making and decision support tools and supporting people in shared decision-making conversations.

  • Work with people with lower activation to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing.

  • Explore and support access to a personal health budget, where appropriate, for their care and support.

  • Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from health coaching

Training Requirements:

  • Must be enrolled in, undertaking or qualified from an appropriate health and coaching training covering topics outlined in the NHS England and NHS Improvement Implementation and Quality Summary Guide, with the training delivered by a training organisation listed by the Personalised Care Institute.
  • Has formal individual and group coaching supervision which must come from a suitably qualified or experienced individual.
  • Adheres to a code of ethics and conduct in line with the NHS England and NHS Improvement Health coaching Implementation and Quality Summary Guide.

PCN Support

Health and Wellbeing Coaches will require a named clinical lead and point of contact at each practice within the network. An induction period should be given and supervision if necessary depending on the level of expertise and experience.
