Job summary

Bromley GP Alliance are looking for self-motivated, forward thinking, qualified clinical pharmacists to join us at this exciting time in the delivery of patient services in Primary Care.

These roles offer direct, face to face patient care and a large degree of autonomy within the Bromley Primary Care Networks (PCN ). Working between the network Practices you will be assessing patients current medications and providing rationale for changes to prescribing to GPs, patients and carers and liaising with colleagues in the Clinical Commissioning Groups (CCG) on the development of area wide prescribing changes.

Main duties of the job

You should be able to provide professional advice and support on the safe prescribing, handling and administration of medicines. This includes support regarding development and implementation of medicines management processes and policies.

Lead on the medicines aspects relating to the care of patients to optimise medicines, reduce medicines-related risks and improve adherence to taking medicines by carrying out medication assessments and reviews in identified patients.

Bromley GP Alliance (BGPA) is a network of 39 Bromley Practices which are working collaboratively to enhance the health and wellbeing of Bromley residents, covering over 310,000 patients, nearly 95% of the Bromley population

Job responsibilities

Main responsibilities

Provide professional advice and support to practices on the safe prescribing, handling and administration of medicines. This includes support regarding development and implementation of medicines management processes and policies.

Lead on the medicines aspects relating to the care of patients in to optimise medicines, reduce medicines-related risks and improve adherence to taking medicines by carrying out medication assessments and reviews in identified patients.

Medicines management with face to face dealings with patients, and carers.

Undertake prescribing for patients within own competency according to the needs of the service.

Resolve day-to-day medicines and prescription issues with healthcare professionals

Help patients to manage their own conditions

Lead practice repeat prescription services and deal with queries from reception staff and patients

Present to Patients any changes to prescribing in line with NICE, best practice or in response to prescribing alerts

Advising patients and carers of the adverse side effect of medication and potential interaction with other medication/treatments

Rationalise repeat prescriptions lists to avoid waste and duplication

Provide reviews to those patients with LTCs or complex health needs, including drug monitoring and risk versus benefit.

Provide on-call pharmacist cover for the practices as per duty rota

Contribute to the management of the prescribing budget through the promotion of safe, cost effective rational prescribing for all residents in care homes

Contribute to minimising the harms associated with the use of medicines, for example by de-prescribing

Link to local and national forums to support embedding best practice.

Qualifications

Essential

  • Masters degree in pharmacy or equivalent qualification pre 2002 Registered with the General Pharmaceutical Council Postgraduate qualification in clinical/community pharmacy or equivalent additional experience Evidence of recent continuing learning/professional development
  • Diploma in clinical/community pharmacy

  • Experience of working in multi-disciplinary teams.
