Job summary

We have an exciting opportunity for a Safeguarding Care Coordinator to support our ambitious growth plans as we continue to expand our Primary Care Network. The successful candidate will be responsible for a coordinated approach to safeguarding within the PCN. This involves identifying those patients at risk in the PCN practices and for implementing and monitoring actions to manage those at risk.

Main duties of the job

We are looking for someone who can work independently whilst integrating well within the clinical and administrative teams. The successful candidate will be appropriately trained in safeguarding and have the skills to undertake the role which will involve highly sensitive and sometimes distressing information.

About us

Procare Health is the GP Federation for Guildford and Waverley Our aim is to support General Practice to work at scale in order to achieve efficiencies of scale whilst respecting the autonomy of individual General Practices. This includes working with other local healthcare providers to develop and provide health services to the benefit of our patients.

We help member practices and their PCNs to work together to jointly address issues which are difficult to resolve individually and offer expertise and skills individual practices find uneconomic to employ. By working at scale, we can ensure the projects we take forward on behalf of our members will allow patients in our area to have more consistency in, as well as choices about, their care.

Procare provide community and district nurses in our area supporting better integration of Primary and Community services. Our ICP area is served by Royal Surrey County Hospital and is located in the Surrey Heartlands ICS area which is one of the original 10 ICS areas in the UK and one of two that has combined Health and Social Care organisations

Job Responsibilities

East Guildford PCN are looking for an enthusiastic and highly organised person to work collaboratively across all 6 practices within the PCN and the linked care homes, providing coordination and administrative support to the GPs, clinical teams and members of the PCN to support patient care and having a keen interest in safeguarding children and adults.

The Care Coordinator post is seen as a critical and evolving role to support the Multi-Disciplinary Teams (MDTs) and the practices within the PCN to deliver effective, proactive and co-ordinated care to patients. Working closely with the PCN Manager and the PCN team they will support the effective workload management of GPs and other clinical colleagues.

They act as a link between the Primacy Care Network, practices and the wider healthcare community and also signpost towards Social Prescribers or Health Coaches. The role requires you to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, occupational therapists and pharmacy technicians.

The Care Coordinator will be safeguarding Level 3 trained and support clinicians to bring together a patient identified care and support needs and ensure all patients have a single personalised care support plan in line with MASH practice standards and best practice, based on what matters to the person.

They will ensure that clinicians have access to the tools they need to develop a personalised care plan. The Care Coordinator will help patients and their carers with queries to allow them to navigate or signpost for their health needs and ensure they have excellent good quality written or verbal information

In more detail, the Care Coordinator will:

  • provide holistic support to the safeguarding leads and discuss patient related concerns, be able to follow appropriate safeguarding procedures to support the GP practices within the PCN
  • arranging and attending Multidisciplinary Team Meetings (MDT), proactively preparing any actions prior to the MDT ensuring all relevant clinicians are present
  • follow up on patients and all forward actions resulting from MDT discussions
  • attend Multi-agency safeguarding Hub (MASH) meetings and work in line with MASH practice standards
  • To take minutes of Safeguarding meetings and distribute these appropriately within the PCN by secure means.
  • co-ordinating and reviewing policies with the safeguarding leads to ensure uniformity and latest policy is followed across the PCN
  • run searches and daily updating of patients on electronic workflow management systems to identify patients with safeguarding concerns and keeping the safeguarding list / register up to date
  • record patient interventions on relevant electronic database systems (e.g. EMIS), safeguarding coding and contribute to report generation, analysis and production
  • follow up on patients coded as child not brought, unvaccinated children etc to identify potential cases of neglect and raise concerns with their usual GP
  • network with local health visitor teams, school nurses, safeguarding leads at school and social services regarding children and young people identified as at risk
  • network with adult services, proactive care team, frailty team for adults identified as at risk
  • ensure regular and consistent communication with the referrer regarding patient progress and any complications or guidance
  • complete S47 reports
  • complete S17 and Child Protection conference information requests
  • maintain a log of S47 and S17 requests and audit that they are completed within the required time frame, identifying any themes to any that do not meet the required response times and implement changes to practice to avoid further breaches
  • offer appropriate support and guidance to patients and their families/carers
  • participate in relevant internal and external working groups and provide project advice, expertise and support when requested relating to safeguarding
  • work closely and in partnership with the other Care Coordinators, Social Prescribing Link Workers and Health Coaches(s) to support the holistic care of East Guildford PCN patients
  • document and monitor aspects of patient co-ordination and service delivery, supporting data collection and audit using the patient administration system
