Job summary
Work as a member of the Day Services and Therapy Team providing a service that focuses on patients goals around empowerment, enablement, maximising wellbeing and self management. Manage a delegated caseload of patients triaged by a qualified therapist to provide support, equipment and advice to patients and their families / carers. Use a rehabilitation model to optimise function, enabling patients to live as fully, safely, and independently as possible within the limitations of advancing disease. Develop alongside colleagues and deliver a model of light touch community support that enables people to access social support and advice at a local venue nearer their home.
Please note. This job is 30 hours per week and pay is pro rated.
Main duties of the job
We are developing our service and have an opportunity to join our Day Services and Therapy team as a Therapy Assistant. The role will require both independent and team working within a variety of settings including the Hospice inpatient unit, and community settings including patients homes. You will have the ability to work alongside the Physiotherapists, Occupational Therapists, Registered Nurse and Complementary therapists in providing services that focuses on patients goals around empowerment, enablement and self-management. The therapists use a rehabilitation model to optimise function. Applicants should have excellent communication skills, demonstrating sensitivity and compassion to patients, families and carers. Experience of working with people with life limiting conditions will be an advantage. You will have experience of working within a multi-disciplinary team, be adaptable, reliable and highly organised. Applicants should also have excellent IT skills.
