Clinical practice
1.
Oversight of the team held clinical caseload of patients living in the community
2.
Ensure a standardized approach to caseload management
3.
Evaluate how best to measure impact on patient care
4.
Take and analyse a clinical history in a relevant succinct and logical manner
5.
Independently assess the patients needs, identify their priorities and work with the patient and family towards achieving them
6.
Demonstrate an in-depth knowledge of the options for achieving control of symptoms in the last weeks of life and ability to advise the primary health care team
7.
Demonstrate an ability to explore the personal resources of families/carers in order to facilitate where possible that patients are cared for in the place of their choice
8.
Support and coach carers at home in managing a relative whose condition is deteriorating
9.
Recognise the impact of caring for dying patients on yourself, internal and external colleagues and offer / seek support
10.
Maintain the competence to safely administer medication in the community including syringe pumps and other competencies in agreed areas.
11.
Recognise and manage palliative care emergencies
12.
Support the team to bring closure to the clinician-patient relationship and provide for a safe transition to another care provider as appropriate
13.
Instigate and support MDT case review meetings.
14.
Investigate and respond to complaints and concerns

15.
Use physical assessment and clinical reasoning skills as part of patient assessment and to support clinical advice, professional communication and documentation.
16.
Perform Independent Prescribing within area of competence once registered as an Independent Prescriber with the Nursing and Midwifery Council/Health and care Professional Council and practice according to local policy.

Use the Recommended Summary Plan for Emergency Care and Treatment (ReSPECT plus) process and form to support the documentation and communication of the patients preferences and clinical recommendations for emergency care and treatment
