Job summary

We have an exciting opportunity for a Consultant or Specialist Grade Doctor in Palliative Medicine to join the St. Peters Hospice medical team. The postholder will join the senior medical team of 4 consultants, working alongside multi-professional colleagues to deliver high quality palliative care to patients and families. They will be working in a supportive and collaborative environment for a much-loved and respected charity serving the vibrant city of Bristol and the surrounding area. The role will have a strong focus on supporting patients in our community, both directly and by working closely with other hospice teams, including provision of support to the hospice advice line. The role will also involve leading work to improve access to services for those with non-cancer conditions. The postholder will contribute to care of inpatients for 10 consultant-led beds.

Main duties of the job

Assessment and management of patients in the community (virtual and face to face) with complex palliative care needs Leadership and support to weekly community team meetings with CNS teams Support to medical advice line Leading patient management on the inpatient unit when cross covering colleagues Provision of 2nd on call support to medical advice line and inpatient unit at a 1 in 5 frequency Active engagement and leadership with quality improvement and service improvement activities Teaching and training of doctors in GP and Specialist Training Schemes

Clinical: To share responsibility with the senior medical team for the advisory support offered to the hospice community services (including domiciliary visiting, attending and leading multi-disciplinary team meetings and outpatient appointments). To share responsibility and engage with the clinical community engagement work which seeks to achieve fair access to clinical services, which includes an approach based on need rather than diagnosis, with a focus on palliative care in non-cancer conditions. To participate in the senior medical cover for the 24-hr Advice Line. To take part in 1 in 5 second on-call rota, defined as a Category B (medium frequency, low intensity) providing senior cover to the inpatient unit and 24 /7 advice line To be prepared to lead the care of hospice inpatients, whether that be short term cover during periods of team leave, or a more defined period of cover for a number of months to undertake additional duties in exceptional circumstances in the case of emergencies and unforeseen circumstances as necessary for the continuity of patient care. To work at all times within the GMC codes of practice Quality Improvement: to take part in the hospice clinical audit programme coordinated by the Patient Safety and Quality Team, which may include participation in local, network wide and national audit.   • Maintenance of full registration and licence to practise with the GMC.
  • MRCP or MRCGP
  • Entry on the GMC Specialist Register via one of the following:
  • Certificate of Completion of Training (CCT) in place at the time of appointment OR
  • Certificate of Eligibility for Specialist Registration in place at the time of employment or likely to be obtained within 6 months
  • Specialist Trainees that do not hold a CCT must be due to be awarded one within 6 months of the interview date.

Desirable

  • Higher academic degree e.g. MD, MSc, PhD
  • Qualification in teaching
  • Additional qualification in Palliative Care

  • Recent experience and familiarity of UK healthcare systems and practices (or equivalent)
  • Completion of a minimum of 12 years medical work (either continuous or in aggregate since obtaining a primary medical qualification, with a minimum of 6 years working in the field of Palliative Medicine (Specialist only)
  • Total of 6 months experience of practising medicine in the community (Specialist only)
  • Can demonstrate achievement of relevant competencies in management of patients with specialist palliative care needs with evidence of appropriate professional development
