Job summary

We have an exciting opportunity for a Consultant in Palliative Medicine to join the St. Peters Hospice medical team. There is also an option for up to 2 additional programmed activities at North Bristol NHS Trust. The post is initially 1 year with a possibility of a substantive post in the future. Applications from Specialist Doctors would also be considered.

The post holder will join the senior medical team of 4 consultants working alongside the multi-professional team members to deliver high quality palliative care to patients and families in the wider Bristol area. They will be working in a supportive and collaborative environment for a much-loved local charity with a strong reputation.

The role will involve a strong focus on supporting patients in our community, both directly and by working closely with our CNS teams. The post holder will be taking part in work to improve access to services for those with non-cancer conditions. The post holder will also contribute to care of inpatients for 10 consultant-led beds. The role will involve contribution to our senior on call rota at 1 in 5 frequency.

Main duties of the job

Key responsibilities:

Assessment and management of patients in the community (virtual and face to face) with complex palliative care needs

Leadership and support to weekly community team meetings with CNS teams

Support to medical advice line

Leading patient management on the inpatient unit when cross covering colleagues

Provision of 2nd on call support to medical advice line and inpatient unit

Active engagement and leadership with quality improvement and service improvement activities

Teaching and training of doctors in GP and Specialist Training Schemes

To share responsibility with the senior medical team for the advisory support offered to the hospice community services (including domiciliary visiting, attending and leading multi-disciplinary team meetings and outpatient appointments).

  • To share responsibility and engage with the clinical community engagement work which seeks to achieve fair access to clinical services, which includes an approach based on need rather than diagnosis, with a focus on palliative care in non-cancer conditions.
  • To participate in the senior medical cover for the 24-hr Advice Line.
  • To take part in 1 in 5 second on-call rota, defined as a Category B (medium frequency, low intensity) providing senior cover to the inpatient unit and 24 /7 advice line
  • To provide cross-cover for members of the senior medical team annual, study and short-term sick leave, which may include review of and leading medical management of hospice inpatients
  • to undertake additional duties in exceptional circumstances in the case of emergencies and unforeseen circumstances as necessary for the continuity of patient care.
  • To work at all times within the GMC codes of practice

Quality Improvement:

  • to take part in the hospice clinical audit programme coordinated by the Patient Safety and Quality Team, which may include participation in local, network wide and national audit.

  • To be part of a learning and safety culture, learning from both what we do well and from incidents that have occurred
