The Social Work Team work alongside patients, families, & carers who need social care support. The role requires working with patients in the inpatient unit & community settings, including patient homes.
The team works closely with families & carers, community nursing teams and the wider hospice services to support people with an incurable disease to have choice and control and to live as independently as possible.
The successful candidate will join a friendly and professional team who make areal difference to patients and their families at the most challenging time.

You will manage your own caseload, therefore the ability to work independently is essential.
We are looking for someone who can support service user with sensitivity and compassion.
Experience of working with people with long term health conditions or approaching the end of their life will be an advantage but not essential along with some previous experience of community working in a health or social care setting.
Main duties of the job
Key responsibilities:
- Manage own caseload.
- Ability to work independently.
- Supporting the Carers Lead with implementation of a program of carers support, including assessment and support planning.
- Whilst this post has a strong Carer focus, there will be some work with patients without carers, dependent on the needs of the service.
- Multi-disciplinary team working.
What we can offer you:
- Salary from £26,152 up to £28,700 annum (FTE), dependant on experience
- Working hours are 22.5 hours per week, 9am-5pm.
- Office location: St Peters Hospice, Brentry, Bristol
- Type of position: Permanent
- No unsociable shifts etc.
You must have a clean driving licence and access to a car as we work across Bristol, South Gloucestershire, and North Somerset.
