Job summary

  • Morecambe Bay Primary Care Collaborative are involved in the delivery of Community Diagnostics, the role is to support patients through a diagnostic pathway. Community Diagnostics is a group of diagnostic test and procedures that require skills and competencies to provide the diagnostics interpret results and direct patients to next stages.

  • To support the Community Diagnostic Services in the delivery of services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.

  • The Health Care Assistant will support and deliver required and specific clinical tests, observations and interventions to patients.

Main duties of the job

  • Assist the service team to deliver effective implementation of management of patients and their needs.

  • Managing the requirements of the patients, following set protocols and delivery of specified actions. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract purposes is also required

  • Aid the service team with implementation of new initiatives in the delivery of Community Diagnostic procedures.

  • Encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working.

  • Recognise the opportunities for working in partnership with the Primary Health and secondary care teams.

  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

  • Deliver opportunistic health promotion using opportunities such as new-patient diagnostics.

  • Assist in the maintaining and developing of services

  • Ability to work flexibly and unsociable hours to meet the demands of the service.

  • 24 Hour Diagnostic Testing i.e. sleep Studies, Blood Pressure and ECG

  • Performance Reporting

  • Support the implementation of evidence-based guidelines in conjunction with the practice nursing team.

  • Participate in an annual appraisal.
