Job summary

Worcester City PCN is looking for a Work and Health Employment Coach to support individuals facing health-related barriers to employment. This role offers a unique opportunity to be part of the WorkWell programme, working with local communities to provide coaching, guidance, and practical support to help people return to work or remain in employment.

Main duties of the job

As part of a wider county-wide network of employment coaches, you will manage a caseload of individuals with physical, mental, or social health challenges affecting their ability to work. You will assess their needs, develop action plans, provide one-to-one coaching, and refer them to relevant health and employment services. No clinical experience is required, and full training will be provided.

Knowledge and Skills

Essential

  • Has an understanding of the drivers and impacts of health inequalities.
  • Experience of partnership working, negotiation and liaison work with other agencies to facilitate progression into work or to retain it
  • Knowledge of the barriers faced by individuals who may be socially excluded and furthest from the labour market.
  • Experience of working on own initiative as well as being an effective team member

Desirable

  • Good Knowledge of the 2010 Equality Act
  • Experience of supporting people into work
  • Working knowledge, understanding and experience of motivating individuals and developing their skills in line with their individual action plans

Qualifications

Essential

  • GCSE grade A to C in English and Maths

Desirable

  • Has an appropriate Health or Employment Coaching qualification

Experience

Essential

  • Has experience of working in a health or employment coaching role, social prescriber or other similar role

  • Has experience of working in, or closely with, Primary Care
