Job summary

The Pharmacy Technician will support the Frailty Team and wider Pharmacy Team and the GPs across the PCN in the medicines optimisation agenda. Medicines optimisation is an approach that seeks to maximise the beneficial clinical outcomes for patients from medicines with an emphasis on safety, governance, professional collaboration and patient engagement. Medicines optimisation is a key focus for PCNs in Redditch. The core central themes moving forward are based upon supporting the PCNs collaboratively to:

  • Integrate care around the patient;

  • Design, Implement and facilitate clinically led medicine optimisation services;

  • Improve productivity and the affordability of services;

  • Advise, engage and train (where appropriate) health care professionals;

  • Improve patient safety and reducing risk;

  • Empowering patients through patient education and engagement;

  • Deliver improved patient outcomes.

Main duties of the job

  • To act as key contact where appropriate, for the PCN to effectively implement or facilitate medicines management initiatives.

  • To undertake appropriate medicine optimisation activities that may include audit with GP practices as directed by the Clinical Director.

  • To implement any changes following the activities above. To communicate those changes to patients and/or healthcare professionals where necessary and appropriate.

  • To assess prescribing within practices, highlight areas for change and implement or facilitate those changes where necessary.

  • To provide information and answer medicines management queries from healthcare professionals and members of the public.

  • To provide training for practice staff which may include clinical systems and audits in response to individual practice needs where required.

  • To monitor prescribing using available tools including EMIS, epact.net and produce reports and feedback for practices

  • Produce and distribute practice support material to assist in the implementation of any local/national targets set

  • To support the implementation of the medicines optimisation strategy through the provision of clinical, administrative and technical expertise.

  • To facilitate the provision of prescribing support to GPs and practice staff to deliver effective medicines optimisation through the implementation and maintenance of high-quality, cost effective prescribing.

About us

Nightingales PCN is a collaboration of 6 GP practices in Redditch extending into Wythall with a shared population of 42,281. Nightingales PCN has an ambition to innovate general practice and build a sustainable model for general practice for the future.

The opportunity to introduce additional roles to complement the existing primary care workforce has been embraced with recent recruitment resulting in the appointments of a combination of roles joining the Additional Roles Team Clinical Pharmacists, Pharmacy Technicians, Physician Associates and FCPs. For new PCN staff, an induction and mentorship approach will be introduced which secures the opportunity to build relationships across the practices and creates effective development and supervision opportunities in order to build necessary expertise and experience.

The precise working pattern will be discussed with the successful applicant and will need to take into account the needs of the PCN to ensure adequate cover Monday to Friday between 8.00am and 8.00pm and Saturday 9am and 5pm, as a team.

  • BTEC in Pharmaceutical Sciences/ QCF Level 3 in Pharmacy Services or equivalent.
  • Registered with General Pharmaceutical Council (GPhC)
  • An appreciation of the nature of primary care prescribing in terms of productivity and affordability of services
  • Ability to provide technical and admin assistance to the Clinical Pharmacist and PCN to implement and review operational strategies, policies and procedures.
  • Ability to provide support and advice to the practices and PCN on meds management and optimization.
  • Excellent interpersonal skills and ability to promote effective communication between Healthcare professionals and patients.
  • Excellent written and verbal communication skills.
  • Is able to monitor, advise and review general medicine optimization issues in core areas for long term conditions.
  • Able to obtain and analyse Emis data to flag-up cost and clinically efficient switches.
  • Able to work under pressure and meet deadlines.
  • Work effectively independently and as a team member.
  • Demonstrates accountability for delivering professional expertise and improving outcomes for patients.

Desirable

  • An appreciation of the nature of GPs and general practices.
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (eg patients).
