Job summary

Droitwich, Ombersley & The Rurals PCN consists of 7 practices, based across South Worcestershire, serving over 53,000 patients. We have an Additional Roles team of 21 healthcare professionals and are looking for an individual with a passion to deliver excellent service in General Practice and in the local communities.

Main duties of the job

Medicines optimisation is an approach that seeks to maximise the beneficial clinical outcomes for patients from medicines with an emphasis on safety, governance, professional collaboration and patient engagement. Medicines optimisation is one of the key focuses of Primary Care Networks in South Worcestershire. The core central themes moving forward are based on supporting the Primary Care Networks collaboratively to:-

Integrate care around the patient

Design, Implement and facilitate clinically led medicine optimisation services

To improve productivity and the affordability of services

Advising, engaging and training (where appropriate) health care professionals

Improving patient safety and reducing risk

Empowering patients through patient education and engagement

Deliver improved patient outcomes

Supporting organisations and networks

SW Healthcare is a GP federation and provider of community-based healthcare services to NHS patients within South Worcestershire. The organisation utilises the excellent clinical expertise that already resides within the local NHS environment and harnesses it with efficient administration pathways to provide patients with more convenient and accessible services. GENERIC RESPONSIBILITIES

To provide technical assistance and administrative support, working with the Medicine Optimisation Pharmacists and clinicians in GP practices to implement and maintain high quality, cost effective changes in prescribing patterns and trends and reducing medicine related risk to patients.

To act as key contact where appropriate, for the Primary Care Networks to effectively implement or facilitate medicines management initiatives.

To undertake appropriate medicine optimisation activities that may include audit with GP practices as directed by the Clinical Director.

To implement any changes following the activities above. To communicate those changes to patients and/or healthcare professionals where necessary and appropriate.

To assess prescribing within practices, highlight areas for change and implement or facilitate those changes where necessary.

To provide information and answer medicines management queries from healthcare professionals and members of the public.

To provide training for practice staff which may include clinical systems and audits in response to individual practice needs where required.

To monitor prescribing using available tools including EMIS, epact.net and produce reports and feedback for practices

Produce and distribute practice support material to assist in the implementation of any local/national targets set

To work with practices, community pharmacies and other primary, secondary and intermediate health care team to improve/develop appropriate medicine optimisation services.

To support the implementation of the medicines optimisation strategy through the provision of clinical, administrative and technical expertise.

To facilitate the provision of prescribing support to GPs and practice staff to deliver effective medicines optimisation through the implementation and maintenance of high-quality, cost effective prescribing.

To provide relevant medicines information and advice to health professionals, CCG staff, patients and carers, including education and training as required.

Committed to CPD ensuring regular updating of clinical knowledge by self-directed learning and attendance at appropriate courses.

  • Registration with General Pharmaceutical Council as a Pharmacy Technician
  • BTEC/NVQ Level 3 Pharmacy services (or equivalent)

Experience

  • Evidence of commitment to continuing professional development
  • Experience working as a qualified, registered pharmacy technician in a community or hospital pharmacy
  • Demonstrate ability to influence and persuade partners and stakeholders of the respective merits of different options, innovations, new opportunities and challenge

  • Experience of working within primary care
