Job summary

Nightingale PCN has an exciting opportunity for a Care Coordinator to join their expanding team. Working across the PCN and wider multidisciplinary teams this is a pivotal role in ensuring all patients receive the best possible care and services available.

The Care Coordinator role will support the PCN General Manager in the coordination and delivery of key PCN Direct Enhanced Service delivery and wider priorities including the coordination of the Frailty cohort of patients, patients living within Care Homes and the broad coordination of the Additional Roles Team. 

The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patient’s journey through Primary Care and the seamless transfer of care across the system. 

The Care Coordinator will possess excellent communication, administration and organisational skills – working directly with the PCN Management Team and the clinicians across the PCN.

Main duties of the job

The successful candidate will provide coordination and navigation for people and their carers across health and care services, alongside Social Prescribing link workers and other primary care roles, ultimately ensuring patients needs are put first and assisting in meeting their personalised care requirements. 

We are looking for someone who has experience of working with healthcare professionals and or previous experience in the NHS or social care and can demonstrate effective partnership / collaborative working as well as possessing knowledge / familiarity with medical terminology.   Experience of data collection and providing monitoring information to assess the impact of services is also key.

Nightingales PCN is a collaboration of 6 GP practices in Redditch extending into Wythall with a shared population of 42,281.

 Nightingales PCN has a shared ambition to innovate general practice and build a sustainable model for general practice for the future.

The opportunity to introduce additional roles to complement the existing primary care workforce has been embraced and recent recruitment has resulted in appointments to the Additional Roles Team including Clinical Pharmacists, Pharmacy Technician, First Contact Physio’s and Physician Associates alongside already established Social Prescribers. For new PCN staff, an induction and mentorship approach will be introduced which secures the opportunity to build relationships across the practices and creates effective development and supervision opportunities in order to build necessary expertise and experience.

Care Coordinators will:

· Provide coordination and navigation for people and their carers across health and care services, alongside Social Prescribing link workers and other primary care roles;

· Support the coordination and delivery of MDTs within the PCN as required;

· Support the coordination of a dedicated multidisciplinary team looking after Care Homes within the PCN area;

· Lead the coordination and navigation of care for a defined frail population across the PCN – proactively supporting and managing patients to remain within their own homes wherever possible – working closely with the wider community team;

· Focus upon patients and provide support necessary fulfil effective personalised care requirements;

· Help patients to manage their needs, answering their queries and supporting them to make appointments – with a focus upon promoting patient independence and autonomy wherever possible;

· Coordination of key meetings (including Care Home residents and frail/vulnerable people living within their own homes. Ensure good communication between professionals and be available as a point of contact between partners;

· Liaise with multi agencies to coordinate pathways of care for patients.

· Provide coordination and navigation for people and their carers across health and care services;

· Support Quality and Outcome Frameworks and other National and Local targets as required.

Contribution to the planning and implementation of services

The post-holder will:

· Apply practice policies, standards and guidance;

· Discuss with other members of the team how the policies, standards and guidelines will affect own work;

· Participate in audit where appropriate;

· Work with the Lead GP to achieve standards of quality, performance standards, without compromising levels of patient healthcare;

· Contribute towards the development and implementation of new standards, policies and procedures that are/will be required of GP Practices now and in the future (as directed by NHS/ DoH/, new legislation etc.).

Communication

The post-holder will:

· Be able to effectively communicate at all levels of the organisation to a variety of health professionals, users and carers, independent and voluntary sector to provide the best outcomes for users of the services.

· Provide interface between hospital, primary, community and social settings, participate where appropriate in clinical meetings relating to patient care and outcomes

· Communicate effectively with patients and carers

· Be able to keep accurate contemporaneous documentation, both written and computerised, inpatient records and will need to be familiar with EMIS Web.

Clinical Governance

· participate and operate within the clinical governance framework for the organisation at all times, incorporating service users and carers, audit, guidelines and risk management;
