Job summary

This is a permanent, part-time role (hours negotiable) working across the six GP practices that make up Nightingales PCN in the area of Redditch, Worcestershire.

Nightingales PCN is a collaboration of 6 GP practices in Redditch extending into Wythall with a shared population of 42,281. Nightingales PCN has an ambition to innovate general practice and build a sustainable model for general practice for the future.

Main duties of the job

Clinical pharmacists will have a key role in supporting delivery of the new Network Contract DES Service specifications. For the new Structured Medications Review and Optimisation requirements this will include tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical-pharmacy led medicines reviews and the positive impact the Clinical Pharmacist will bring to the wider MDT.

· Working within the practice based team to undertake medication reviews particularly in high risk groups such as:

- Frail elderly

- Poly-pharmacy

- Renal impairment

- Hepatic impairment

- Asthma, diabetes, hypertension and Rheumatoid and DMARD drug monitoring

- Substance misuse

- Patients on high risk medicines

- STOPP/START identified patients

- Revolving door Hospital admissions

· Management support for the dispensary including: profitability and stock control; negotiations with drug company representatives and wholesalers; and organisational management including review of SOPS (Standard Operating Procedures) and compliance issues;

· Improve prescribing practice through educational support for all prescribers within the practice;

· Lead on where changes in evidence require changes in prescribing across the patient population, e.g. where a drug is withdrawn or indications change;

· Liaise with colleagues in community pharmacy to align support for medicines adherence;

· Support improvements in clinical care through practice based audit and implementing change;

· Prescribing advice to prescribers in practice e.g. temporary non-availability of drugs;

· Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines;

· Lead for medicines optimisation scheme compliance, including care home support; discharge reviews; medication reviews; and QIPP;

· Has an awareness of the breadth of common acute and long term conditions that are likely to be seen in a general medical practice.

Essential

  • Min 2 4 years post graduate (2 years experience working in general practice or 4 years in other comparable healthcare setting).
  • Experience of public health promotion/high risk medicines monitoring/lead on antimicrobial stewardship/safe and effective repeat prescribing.
  • Good working knowledge of QOF and contribution to QOF delivery.
  • Good working knowledge of and role in Enhanced Services.
  • An appreciation of the nature of GPs and general practices.
  • Able to carry out Pharmaceutical Needs Assessment.
  • Able to conduct clinical medicine reviews, reduce waste through poly-pharmacy and over prescribing, reconcile and optimize medications.
  • An appreciation of the nature of primary care prescribing concepts of rational prescribing and strategies for improving prescribing.
  • Excellent interpersonal influencing and negotiating skills.
  • Excellent written and verbal communication skills.
  • Is able to plan, manage, monitor, advise and review general medicine optimization issues in core areas for long term conditions.
  • Able to obtain and analyse complex technical information.
  • Recognizes priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
  • Able to work under pressure and meet deadlines.
  • Gain acceptance for recommendations and influence, motivate, persuade the audience to comply with the recommendations, agreed course of action where there may be significant barriers.
