Job summary
Medicines optimisation is an approach that seeks to
maximise the beneficial clinical outcomes for patients from medicines with an
emphasis on safety, governance, professional collaboration and patient
engagement. Medicines optimisation is one of the key focuses of Primary Care
Networks.
We are looking for a motivated, enthusiastic and professional Pharmacy
Technician with a good clinical background, excellent communication and
presentation skills. You will be joining the pharmacy team within Droitwich/Ombersley primary care network. Applicants will be computer literate, organised and
confident in working independently as and part of a wider team and you must be
able to prioritise workload.

The successful applicant will support the Primary
Care Network to collaboratively:
Integrate care around the patient
Design, implement and facilitate clinically led
medicine optimisation services
Improve productivity and the affordability of
services
Advise, engage and train health care professionals
Improve patient safety and reducing risk
Empower patients through patient education and
engagement
Deliver improved patient outcomes
Support organisations and networks
About us
Droitwich/Ombersley Primary Care Network comprises six GP Practices made up of three practices in Droitwich, one in Ombersley, one in Great Witley and one in Knightwick. We
have established a strong working relationship between practices and strive to
provide excellent quality of care for our population.

GENERIC RESPONSIBILITIES
To
provide technical assistance and administrative support, working with the
Medicine Optimisation Pharmacists and clinicians in GP practices to implement
and maintain high quality, cost effective changes in prescribing patterns and trends
and reducing medicine related risk to patients.
To act
as key contact where appropriate, for the Primary Care Networks to effectively implement
or facilitate medicines management initiatives.
To
undertake appropriate medicine optimisation activities that may include audit with
GP practices as directed by the Clinical Director.
To implement
any changes following the activities above. To communicate those changes to
patients and/or healthcare professionals where necessary and appropriate.
To
assess prescribing within practices, highlight areas for change and implement
or facilitate those changes where necessary.
To
provide information and answer medicines management queries from healthcare
professionals and members of the public.
To
provide training for practice staff which may include clinical systems and
audits in response to individual practice needs where required.
To
monitor prescribing using available tools including EMIS, epact.net and produce
reports and feedback for practices
Produce
and distribute practice support material to assist in the implementation of any
local/national targets set
