KEY
RESPONSIBILITIES:
Performance
Working in accordance
with the treatment matching model regarding client segmentation.
To undertake individual
assessments on clients entering the service.
To be aware of, and
contribute to, the achievement of Key Performance Indicators (KPIs) that Open
Road is required to deliver.
To be responsible for
accurately recording and maintaining client data, records and information
onto a case management system, and to maintain up to date and professional
case files in accordance with Inclusion Health Cares policy and procedures
MAIN TASKS
Clinical Governance
To provide effective activity recovery
management, ensuring all areas of risk, problematic behaviour, housing,
social functioning, employability, psychological and physical health are
addressed.
To devise and develop with the client, their
individualised and person-centred activity plan, ensuring that all their
needs are identified and met. The plan will be activity focused and strengths
based, incorporating, and identifying all resources available to the client to
support their recovery journey. Day to Day Operations
To work with partnership organisations to
develop and sustain effective and comprehensive treatment pathways to ensure
delivery of a cohesive service provision to all clients.
To arrange and co-ordinate clients individual
activity/interventions sessions with relevant workers and other service
providers.
To assist the development of clients individual
recovery capital and support the clients in accessing support within the
wider recovery community, including actively linking clients to peer support
and mutual aid.
To
promote and support clients into both abstinence based and medically assisted
forms of recovery.
To develop a therapeutic relationship with
clients, and to deliver low intensity Psychosocial Structured Interventions
(PSI) in line with best practice and partner agencies.
To deliver harm reduction interventions,
information, and advice to all clients, in line with national best practice
and local guidance. This will include needle exchange, Blood Borne Virus
(BBV) and other health interventions linked to substance misuse.
To actively case manage and key work clients,
undertaking regular reviews of the activity plan, risk assessment and
Outcomes.
To ensure that all safeguarding issues including
child protection and protection of vulnerable adults, are dealt with in
accordance with Inclusion Health Cares policy and procedures, and in line
with local and national statutory requirements.
To maintain and develop productive working
relationships with management, team members, volunteers, and clients, and to
maintain personal and professional boundaries, and work within Inclusion
Health Cares clinical framework always, including confidentiality.
To attend team meetings, supervision and other
events organised by Inclusion Health Care.
To facilitate appropriate and effective client
consultation, feedback, and involvement in all your work, to ensure effective
communication regarding improvement of services.
To actively participate in individual
supervision and appraisal.
To actively participate in continued
professional development.
To work flexibly across operational sites as
required Health & Safety Adhere to all health and safety policies of the
organisation and take responsibility for the health and safety of yourself
and others.

While seeking treatment, patients entrust us with or allow us to
gather, sensitive information in relation to their health and other
matters. They do so in confidence and
have the right to expect that staff will respect their privacy and act appropriately.
In the performance of the duties outlined in this job description,
the post-holder may have access to confidential information relating to
patients and their carers, practice staff, and other healthcare workers. They may also have access to information
relating to the practice as a business organisation.All such
information from any source is to be regarded as strictly confidential.
Information relating to patients, carers, colleagues, other
healthcare workers, or the business of the practice may only be divulged to
authorised persons in accordance with the practice policies and procedures
relating to confidentiality and the protection of personal and sensitive data.
PHYSICAL ELEMENT OF THE ROLE

post requires some physical effort depending on the area of work.
SAFEGUARDING RESPONSIBILITIES
Inclusion takes the issues of Safeguarding Children,
Adults and addressing domestic violence very seriously. All employees have a
responsibility to support Inclusion in our duties by:
o Attending
mandatory training on Safeguarding children and adults.
o Making
sure they are familiar with their requirements under relevant legislation; o
Adhering to all relevant national and local policies, procedures, practice guidance
(e.g., LSCB Child Protection Procedures and Practice Guidance) and
professional codes.
o Reporting
any concerns to the appropriate authority/Safeguarding Lead
INFECTION CONTROL
To support Inclusion in achieving a reduction in Health
Care associated infections i.e.
o to
carry out duties placed on employees by the Health Act 2006.
o to
be familiar with, and comply with Trust Policies / guidelines on infection
control; and
o to
attend all mandatory training in relation to infection control.
RISK MANAGEMENT / HEALTH
AND SAFETY
Employees must be aware of
the responsibilities placed on them under the Health & Safety at Work Act
1974, ensure that agreed safety procedures are carried out and maintain a
safe environment for employees, patients, and visitors.
SMOKING AT WORK
Inclusion operates a non-smoking
policy. Employees are not permitted to smoke anywhere within the premises or smoke
during working time and when representing Inclusion.
POLICIES AND PROCEDURES
Inclusion employees are expected
to follow policies, procedures, and guidance as well as professional standards
and guidelines. Copies of Inclusions policies can be accessed via Trello,
Staff Handbook, or via your Manager/ HR Department
