services within Leicester prison, Arnold Lodge (medium secure psychiatric
hospital) and substance misuse services and could provide additional
opportunities working within these settings should that be of interest.

Main duties of the job
JOB SUMMARY
We are seeking a GP to become a key member of our
primary care team, working in both Inclusion Practice, which is a GP practice
for people who are homeless, and Assist Practice, which provides for asylum
seekers and refugees.

patients, asylum seekers and other marginalised groups
As part of the
role, you should be able to:
Making
professional, autonomous decisions in relation to presenting problems, whether
self-referred or referred from other health care workers within or outside the
organisation.
Assessing the
health care needs of patients with undifferentiated and undiagnosed problems.
Be confident in recognising and managing safeguarding concerns.
Use
interpreters on a regular basis to perform consultations.

Job responsibilities
JOB PURPOSE:
The main objective of
this multi-skilled role isto work as a General
Practitioner who can practise autonomously in delivering care within their scope of practice to our homeless
patients, asylum seekers and other marginalised groups
RESPONSIBLE
FOR:
o In
accordance with the Practice timetable, as agreed, the post-holder will make
them available to undertake a variety of duties including surgery
consultations, telephone consultations and queries, visiting patients at
home, checking and signing repeat prescriptions and dealing with tasks,
paperwork, reports and correspondence in a timely fashion.
o To
provide 15 minute consultations for booked appointments and work flexibly
according to patient need.
o To
manage emergencies should they present
o Making
professional, autonomous decisions in relation to presenting problems,
whether self-referred or referred from other health care workers within or
outside the organisation.
o Assessing
the health care needs of patients with undifferentiated and undiagnosed
problems.
o Screening
patients for disease risk factors and early signs of illness.
o In
consultation with patients and in line with current Practice disease
management protocols, developing care plans for health.
o Providing
advice and health education.
o Arranging
follow-up as required and referring to other care providers within specialist
primary care, secondary care and partnership organisations as appropriate.
o Recording
clear and contemporaneous consultation notes to agreed standards.
o Collecting
data for audit purposes and complying with QOF requirements.
o Compiling
and issuing computer-generated acute and repeat prescriptions.
o Prescribing
in accordance with the Practice prescribing formulary (or generically)
whenever this is clinically appropriate.
o To
respond to requests for brief letters for patients regarding their health
needs and accommodation concerns. To adhere to practice policy regarding
charges for non NHS work, the fees for which will be retained by the service.
o To
support the other members of the primary care team especially those
developing new and extended skills. To participate fully in practice meetings
and learning activities.
o To
use interpreter services whenever necessary to communicate effectively with
the patient.
o To
support the practice on-call rota (12:30-14:00) as determined by the days and
hours worked.
o To
be reflective on ones own personal health and mental wellbeing at all times.
To recognise the psychologically distressing nature of many of our patients
histories. All staff are expected to engage in clinical supervision sessions.
o To
participate in the process of audit and significant event recording.
o Ensure confidentiality is maintained at all times as per
Caldicott requirements.
o In
general the post-holder will be expected to undertake all the normal duties
and responsibilities associated with a GP working within primary care.
