We
are looking to appoint a Bereavement Support Coordinator to join the Bereavement
Service providing support to people living in and around the Dudley Borough.
As a
Bereavement Support Coordinator, you will work as part of the Patient and
Family Support Services providing information and support for bereaved
individuals and their families. The role includes assessing referrals,
delivering skillful and compassionate support to individuals, and groups. Alongside supervising volunteers, you will be required to assist
in education and training sessions for volunteers and others who support
bereaved people.
Main duties of the job
Coordinating the delivery and administration
of hospice bereavement care.
Providing post bereavement support
to family, friends
and carers of hospice
patients and some pre-bereavement support.
Delivering support
in individual, family,
and groups settings
as appropriate.
The supervision of volunteers delivering bereavement care.
Working
closely with all members of the hospice team, in identification of clients and delivery
of service.
Supporting the delivery of educational commitments.
Delivery of post-bereavement support
and advice with individuals and
families.
Provision of a supportive approach, which is appropriate to client needs.
Identification of cases appropriate for allocation to Bereavement Volunteers.
Organisation and administration of the
referral of clients, who may need further support
from external agencies.
Maintaining working
relationships with other external services
and ensure effective liaison.
