Job summary

Advanced nurse practitioners work autonomously within the practice using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or other general practice presentations.

Please note, the employer will be Langley Corner Surgery.

Main duties of the job

Advanced Nurse Practitioner's are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

ANP's in this role will work with all patients and be required to manage and support housebound patients with complex needs via weekly MDT meetings.

The partners have been and still remain Medical Officers to a number of Care Homes in the community.

Job responsibilities

MAIN DUTIES AND RESPONSIBILITIES

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

1. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

2. Implement and evaluate individual specialised treatment plans for chronic disease patients

3. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

4. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

5. Undertake the collection of pathological specimens

6. Request pathology services as necessary

7. Process and interpret pathology and other test results as required

8. Maintain accurate clinical records in conjunction with extant legislation

9. Ensure SNOMED CT codes are used effectively

10. Prioritise health issues and intervene appropriately

11. Support the team in dealing with clinical emergencies

12. Recognise, assess and refer patients presenting with mental health needs

13. Be an extended and supplementary prescriber, adhering to guidance

14. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

15. Contribute to practice targets (QOF, etc.), complying with local and regional guidance

16. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

17. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

18. Support the clinical team with all safeguarding matters in accordance with local and national policies

19. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

20. Deliver opportunistic health promotion where appropriate

21. Support development and training of other colleagues e.g. medical students, GP trainees.

Supervision

The post holder will have access to appropriate clinical supervision and an appropriate named individual in the GP Practice to provide general advice and support on a day-to-day basis. We offer a daily lunchtime meeting where you will be able to discuss patients and seek colleague advice.

  • Registered Nurse with Nursing and Midwifery Council
  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet

Experience

Essential

  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
  • Essential clinical skills including phlebotomy, ECGs, urine analysis and swab taking.
  • Working knowledge of Microsoft and GP practice and prescribing data monitoring systems (We use SystmOne for our clinical system).
