Job summary
This role will support the PCN Pharmacy team in the smooth co-ordination of care across the North Tandridge Primary Care Network.
The ambition is to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.
The Coordinator will support the Clinical Pharmacists in identifying and managing a caseload of patients to ensure effective and efficient use of medicines.
Main duties of the job
The post holder will work as part of a multi-disciplinary team and this role involves provision of technical support for the pharmacy service to ensure professional operational standards and excellence in pharmaceutical care and safety are maintained.
The role will support the practices within North Tandridge PCN to ensure the delivery of safe, effective and efficient systems for medicines related clinical correspondence; repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

Primary
duties and areas of responsibility
- To work
across a Primary Care Network supporting a wide range of patients of all ages
alongside the Pharmacy Team.
- Identify
patients that may need support by receiving information about transfers of care
(including hospital admissions and discharges) and from internal practice
intelligence.
- Support the coordination of appointments for the Pharmacy Team to
ensure achievement of Impact and Investment Fund indicators including
Structured Medication Reviews
- Help
patients understand their condition by liaising with clinical colleagues,
especially the practice pharmacists, regarding their medication. Aim for
patients to have specific instructions regarding their medication and
understand how they access repeat prescriptions and reviews.
- With the
help of relevant clinical colleagues, develop a care plan to address patients
personal health care needs. Ensure care plans are maintained, updated, and
uploaded to all relevant systems for sharing with other providers, including EMIS
and ShareMyCare.
- With the
help of relevant clinical colleagues, educate patients (and if applicable and
if appropriate consent is in place, their carers or family) about their
condition and medication, and give them specific instructions.
- Signpost team members, service users
and carers to relevant services including the CPCS Service
- Liaising
with Acute Trusts, Hospices, Community and Social Care providers as required
Record Keeping and General
Responsibilities
- Keeping
up to date records in all GP clinical systems across the PCN, including data
that can be reported on and shared with the PCN for outcomes, on a monthly
basis
- To use
read codes to tag those patients identified for interventions and must be
placed on the patients record, so that activity and metrics associated with
these patients can be tracked over time by the PCN to monitor outcomes and
provide data for proof of concept
- Uploading
and maintaining patient care plans to EMIS and other relevant systems as
necessary
- To
actively engage with the practice teams within the PCN, ensure effective
liaison with all PCN staff and contribute to the overall aims of the PCN
- Ensuring
templates are completed by professionals in order to provide accurate data, by
encouraging there use and auditing regularly to ensure adherence
- To
support the Pharmacist team to collate monthly data from each Practice within the
PCN
- To
attend and contribute to relevant meetings
