Job summary
This is an exciting opportunity for the successful candidate to join
the Haywards Heath Central Primary Care Network (PCN) team in supporting a wide
range of patients of all ages. This post is part of the Additional Roles
Reimbursement Scheme for Primary Care Networks funded by NHS England.
Whilst General Practice has some challenging times ahead,
this role provides an exciting and innovative opportunity for someone who wants
to make a real difference in the communities they serve. You will work
autonomously supporting patients across the PCN, providing enhanced clinical
assessment and treatment skills to provide first point of contact for patients
presenting with undifferentiated, undiagnosed problems.
Main duties of the job
You will work closely with clinicians from all of the Practices across Haywards Heath Central PCN and will play a vital role in increasing capacity for all appointments including home visits.
The successful candidate will be provided with excellent support, the opportunity for development in the role, and will be clinically supervised by a General Practitioner.

Clinical
Responsibilities
- To
provide assessment, diagnosis and treatment at first point of contact by
attending to patients according to patients needs either in the Practice or
patients homes.
- Work
with practice teams to ensure the process for referral of urgent on the day
patients is smooth and appropriate patients are referred making full use of the
practitioners skills.
- To
accurately triage and prioritise patients, including in emergency situations,
demonstrating the use of a variety of techniques to elicit the history of an
event/illness, including past medical and drug history.
- To
evaluate clinical information from examination and history taking and initiate
appropriate treatment and/or referral.
- To
undertake and interpret diagnostics and take appropriate action to manage
patient care, whilst distinguishing between abnormal and normal findings in
order to relate them to a possible diagnosis. To use broad clinical guidelines
(NICE etc) relating them to individual cases, providing assistance and feedback
to the organisation and others on how these should be interpreted.
