Job summary

North Tandridge PCN are looking for a Network Coordinator. This role will be based with North Tandridge PCN supporting the Clinical Director, Network Manager and practices as they develop as a Primary Care Network. The post holder will be fully supported by the wider ABC team to ensure the PCN is able to develop and thrive.

This role provides the ‘on the ground’ presence with links to the full services that ABC offers to PCNs: 

  • Recruitment and Employment
  • Financial Management and Bookkeeping
  • Project Management
  • Strategic planning and support
  • Leadership and team development. 

ABC will also support the post holder by sharing national guidance and best practice and linking them to the wider network of PCNs across East Surrey and West Sussex.

Main duties of the job

Provide operational and administrative support to the Clinical Director, Network Manager and Primary Care Network practices to enable it to deliver the PCN Direct Enhanced Service framework (DES), progress through the maturity matrix and meet the strategic aims set out by the practices.

To work with ABC’s Network Development Manager and team to ensure that the PCN Additional Roles are well supported on a day to day basis and the network is able to make full use of their potential.

To work with the PCN Board and all network practices to help develop and embed joint systems of working both between practices and with other system providers (acute, community, mental health and third sector).

Alliance for Better Care CIC (ABC) is the federation of 47 East Surrey, Crawley, Horsham, and Mid Sussex GP practices, established in 2014, and now comprising twelve Primary Care Networks. ABC provide employment and management support to the North Tandridge Primary Care Network comprising the following practices:

  • Administration of communication between and external to the network in liaison with the Clinical Director and Network Manager, establishing shared documents, planning meetings and being in regular contact with PCN Practice Managers

Production and delivery of the Network Business Plan

With support from the ABC team produce and work to an annual business plan linked to a longer term strategic plan that includes both DES requirements and local projects. Ensure the plan is regularly updated and can be reviewed by the whole PCN.

Project Management of Identified Projects

Planning of projects

• Establish initial project brief, asking key questions of those involved and doing any background research

• Present in a format that is clear and understood by all stakeholders to ensure buy in and engagement

• Work with others in the team to estimate time and resource required for delivery and how these fit with other priorities

• Identify stakeholders and ensure build effective working relationships with individuals and groups

• Set measures against which the success of the project will be addressed

Project Delivery

• Establish necessary mechanisms for delivery of project e.g. working groups, learning sets, workshops

• Design project plan and reporting frequency

• Support project delivery as required to ensure objectives can be achieved.

Project Review 

• Ensure data collected throughout project available for objective assessment of outcomes

• Write up report of project and impact for circulation as appropriate including lessons learnt for future projects

• Ensure project is well communicated at close as well as throughout the duration
