Job summary

Populations are changing and patients are living longer with complex care needs. This project aims to create a frailty clinical pathway that all practices within Haywards Heath Central, Haywards Heath Villages and Burgess Hill and Villages PCNs can refer into.

The Primary Care Networks (PCNs) in Mid Sussex are made up of a number of practices, as follows:

Haywards Heath Central PCN:

  • Newtons Practice
  • Dolphins Practice

Haywards Heath Villages PCN:

  • Lindfield Medical Centre
  • Cuckfield Medical Practice and The Vale Surgery
  • Ouse Valley Practice
  • Northlands Wood Practice

Burgess Hill and Villages PCN:

  • Mid Sussex Health Care
  • Silverdale Surgery
  • Meadows Surgery
  • The Brow Medical Centre

The combined patient population of these Primary Care Networks is 114k.

Main duties of the job

Having an Anticipatory Care service that is well triaged by practices aims to create a better patient experience, a better way of working collaboratively and a reduction in unplanned admissions to hospital. This service will be available 4-5 sessions a week (days to be confirmed).The Frailty General Practitioner will be responsible for providing clinical leadership to the Anticipatory Care Hub team. It is therefore desirable you should also have ‘experience of or qualifications in clinical supervision/education’ but not essential as training can be given.

The frailty hub is a project being led by Alliance for Better Care and the role of the Frailty General Practitioner is to care clinically for a cohort of patients as identified by the individual practices within the Mid Sussex PCNs. This role will include seeing patients in their own homes as well as visits to care homes where appropriate.

Job responsibilities

The Frailty General Practitioner will be responsible for the following (there may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels):

  • using clinical skills to diagnose and treat patients accordingly
  • liaising with other professionals responsible for the patient’s care
  • liaising with secondary care, community care, social care, mental health services and other local providers
  • developing a care plan for those patients at risk of hospital admission (including ReSPECT forms where appropriate)
  • making referrals to other teams as necessary
  • entry in medical records using Ardens Templates and accurate clinical coding
  • The delivery of highly effective medical care to the entitled population
  • Generic prescribing adhering to local and national guidance
  • Effective management of long-term conditions
  • Processing of administration in a timely manner, including referrals, repeat prescription requests and other associated administrative tasks
  • Undertake telephone triage and prioritise home visits accordingly
  • Maintain accurate clinical records in conjunction with good practice, policy and guidance
  • Working collaboratively across the PCN with all practices and partner organisations and agencies
  • Discussing patients at both PCN level MDTs and at Practice level MDTs
  • Adhere to best practice recommended through clinical guidelines and the audit process
  • Contribute to the successful implementation of continuous improvement and quality initiatives with the Practices and PCN

There will be a period of induction across the PCN, the Practice Manager’s will ensure you have induction at each practice.

All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments. Effective communication is essential and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner.

Delivery

The project will be delivered in two phases, an initial brief planning phase, followed by an implementation and monitoring phase that will review the impact on practices & patients.

By the end of the project a summary report will be produced that addresses effectiveness and possible continuation of the service.

This report will be available to other practices, PCNs and the commissioner.

The GP Federation (ABC) will support delivery of the project throughout with additional help from Mid-Downs Training Hub regarding workforce.

Each participating practice will designate a responsible GP partner to lead on this project. A project working group will meet at least bi-monthly to monitor progress and receive interim reports.

The security of all practice is the responsibility of all personnel.

Across the Care Collaborative PCN staff are required to dress appropriately for their role.

Leave

Leave will be authorised by your Clinical Supervisor who will ensure you are afforded the opportunity to take a minimum of 30 days (pro-rata) leave within this one-year contract.
