Job summary
Haywards Heath Villages Primary Care Network is looking for a Clinical Pharmacist. Practices within the network are already experienced in working with Clinical Pharmacists and this role provides a great opportunity to join a committed team as well as having access to national training through the NHS England Clinical Training Pathway and be part of a local network of clinical pharmacists.
The successful candidate will be based mostly at one practice site but there will be an expectation that cross-cover will be provided to all three network practices as needed. Some home working may be necessary. Time will also be dedicated to training and networking. Main practice based to be confirmed on appointment.
Full time/part time will be considered.
This is a fixed term contract until 31st March 2024.
Main duties of the job
The post holder will join the growing pharmacist team for the Haywards Heath Villages PCN.
This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England and includes funded access to the Clinical Pharmacist Training Pathway for any further modules required to complete the pathway.
The post holder is a Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinical Pharmacist who will develop, manage and mentor them.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).
