Job summary

This is an exciting opportunity for a Clinical Pharmacist to join the Care Collaborative Primary Care Network. The post holder will report to the Senior Clinical Pharmacist and GP Clinical Lead within the Primary Care Network for clinical supervision. They will also have strong working relationships with the Practice Managers and other clinicians in the network.

Alliance for Better Care CIC will provide consistent employment support throughout the appointment.

As part of the clinical duties in both roles, the successful candidates will join the multi-disciplinary staff in providing comprehensive care and will work with patients, carers, and families.

ABC as employer will provide support and ensure access to the NHSE England training programme as well as a local network of Clinical Pharmacists working in general practice.

This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England and includes funded access to the Clinical Pharmacist Training Pathway for any further modules required to complete the pathway.

Main duties of the job

The post holder is a Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinical Pharmacist.

The post holder will work as part of a multi-disciplinary team in a patient-facing role.

This is a fixed term role until 31st March 2024.

Full and part time considered.

PCN Care Home/Anticipatory Care Pharmacist Responsibilities:

  • To promote high quality, safe, evidence based and cost-effective prescribing in a care home setting and ensure prescribing conforms to relevant legislation
  • Responsible for ensuring the development and delivery of the medicines optimisation agenda in Care Homes/Anticipatory Care and liaising with partners and other stakeholders on a regular basis
  • Ensuring that relevant activity and outcome data is recorded and reported in a timely fashion
  • To attend the Anticipatory Care Home MDT weekly, supporting the multi-disciplinary team
  • To work closely with the Senior PCN Clinical Pharmacist and the Anticipatory Care GP
  • Ensuring the PCN meet the requirement as specified in the EHCH (Enhanced Health in Care Homes) Specification and the Anticipatory Care Specification
  • To answer care home medicines queries, both urgent and non-urgent to a variety of people (by email, telephone, verbal, written)
  • Plan, organise and conduct clinical and structured medication reviews with the aim of reducing unnecessary polypharmacy, either remotely or/and face to face
  • Work with autonomy organising and planning work projects, meetings and training sessions for care home and clinical staff
  • To liaise with care homes and EHCH colleagues to provide medicine support, information and training
  • Support the EHCH team to develop strong multidisciplinary working relationships with stakeholders and partners, e.g. Care Homes, GP Surgeries, Community Pharmacists, PCN Pharmacists, Local Authority, Commissioners and Clinical Governance Leads
  • Plan and conduct structured medication reviews (SMRs), cost-effective prescribing and de-prescribing of medications
  • Lead audits across the PCN to improve quality

PCN Primary Care Pharmacist responsibilities:

  • Support practices with clinical and structured medication reviews and prioritise these based on PCN goals
  • Deal with a wide range of medication queries from patients and staff, face to face or by telephone
  • Support and be involved with management of common/minor/self-limiting ailments via triage
  • Ensure patients receive timely post discharge medication reconciliation and correct transcribing of medication from clinic letters
  • Participate in service development in the PCN practices e.g. patient information leaflets, treatment pathways, prescribing guidance
  • Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the relevant practice team
  • Monitor practice prescribing against the local health economy’s RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs)
  • Support other healthcare professionals in the primary care team with medication optimisation and proactive support for teaching new members of staff
  • Provide leadership to non-clinical staff with dealing with repeat prescriptions and queries from patients
