Job summary

This new role will be based with South Crawley Primary Care Network supporting the Clinical Director and practices as they develop as a Primary Care Network. The post holder will be fully supported by the wider ABC team to ensure the PCN is able to develop and thrive.

The post holder will work with ABC's Network Development Manager and team to ensure that the PCN Additional Roles are well supported on a day to day basis and the network is able to make full use of their potential.

This contract is until 31st March 2024.

Main duties of the job

Provide operational and administrative support to the Clinical Director and Primary Care Network practices to enable it to deliver the PCN Direct Enhanced Service framework (DES), progress through the maturity matrix and meet the strategic aims set out by the practices.

To work with the PCN Board and all network practices to help develop and embed joint systems of working both between practices and with other system providers (acute, community, mental health and third sector).

Recruitment and Employment

Financial Management and Bookkeeping

Project Management

Strategic planning and support

Leadership and team development

A few Core Duties and Responsibilities

Communication across the network

Administration of communication between and external to the network in liaison with the Clinical Director, establishing shared documents, minuting and planning meetings and being in regular contact with PCN Practice Managers.

Production and delivery of the Network Business Plan

With support from the ABC team produce and work to an annual business plan linked to a longer term strategic plan that includes both DES requirements and local projects. Ensure the plan is regularly updated and can be reviewed by the whole PCN.

Development of the PCN Team

With support from ABC development of the PCN employed roles (Pharmacists, Social Prescribers, Physicians Associates), ensuring that they can work effectively across all practices and opportunities for getting the most from the staff are shared between practices.

Awareness of opportunities for the PCN

With the support of the Network Development Manager gain knowledge of opportunities available locally and nationally for the PCN, liaising with PCN members and writing proposals or bids to progress these where appropriate.
