Job summary

Alliance for Better Care is working with our partners in the East Surrey place to recruit roles to support our innovative Aging Well programme. This programme aims to try out new approaches and services to ensure that care for everyone in East Surrey as they get older is preventative, holistic and enables them to live at home as long as possible and die well.

We are looking for a Project Manager and would be very keen to speak to any interested candidates as can be flexible in how the role is delivered. Main duties of the job

Ageing Well Programme

The shared purpose of this multi-agency programme is to:

To work together on transformational projects across our organisational boundaries, sharing best practice and expertise and keeping the patient at the centre so that care for people as they get older is preventative, holistic, has less hand offs and enables them to live at home as long as possible and die well.

The purpose of this role is to provide project management support to the programme, both overall steering group and projects and enable those working in the groups to achieve their desired outcomes.

Main Duties and Responsibilities

Establishing Rigorous Project Management

Produce and own the overall programme plan for the Ageing Well Programme to include the following sub groups:

Primary Prevention

Anticipatory Care Hubs

Frailty Virtual Wards

Ensure that working and steering groups have appropriate information to understand what they are aiming for and whether they are on track.

Work in an agile way to keep action notes from meetings and share documents through MS Teams to minimize the overall administrative burden of the programme.

Liaison with partners

Linking with the CCG and providers involved in the programme is key to this role, key relationships are as follows:

CCG: Head of Transformation and Delivery, Place PHM Programme Lead, Senior Programme Lead, Head of Primary Care, Clinical Leads

First Community: Chief Operating Officer & Ageing Well Programme Director, Head of UCR, Transformation Lead

SASH: Integrated Discharge Team Lead, Geriatricians

All members of the Ageing Well Steering Group and Teams

Financial Reporting

To ensure that any project budgets are tracked and necessary information is provided to the ABC finance team, to share reporting as required.

Recruitment

In liaison with the People Services team at ABC to progress any further recruitment required within the programme or to link with HR in other organisations and gain updates on recruitment.

Analysis for Improvement

To work with the Project Analyst to ensure that we have data to show impact of improvement cycles made within the programme.

Quality Improvement Techniques

To attend training and develop use of QI techniques in support of the transformation programme such as PDSA cycles, KanBan and Lean Thinking methodologies.

This is a varied role and you may be asked to take on other responsibilities in line with the seniority of the post.
