The successful candidate will be based at one of our geographical hubs in either Rugby, Nuneaton or Stratford-Upon-Avon and will provide support across Warwickshire.
We have an exciting opportunity for a Learning Disability Practitioner to join Connect for Health Warwickshires School Nursing Service. Your role will be to provide experience and expertise to children, young people with learning disability. You will also support your colleagues to expand their knowledge of how best to support individuals with learning disabilities. You will be required to facilitate and deliver a range of assessments, brief interventions, proactive public health initiatives and will be able to work both independently and as part of a multi-agency team around the family. The post holders caseload will consist of children and young people with learning disabilities across mainstream schools. You will also further develop our partnership with SEND Services across Warwickshire.
Main duties of the job
We are looking for someone who is passionate about improving the lives of children and young people. The successful candidate will have experience of supporting children, young people and families with SEND and will be able to adapt the care they provide to meet individual needs.
You must possess excellent communication skills, with the ability to establish and maintain positive relationships with partners and service users. You will be able to maintain accurate health records.
You will have a full UK driving license and access to your own transport. You will need to be able to travel across Warwickshire.

- NNEB (Nursery Nurse) or BTEC higher diploma or foundation degree (Assistant Practitioner) or equivalent skills/knowledge and experience

- Extensive experience of working with children and young people with learning disabilities in educational settings
- Understanding of health issues that affect children, young people and their families.
- Knowledge of child development
- Experience of assessing health needs and providing interventions on an individual and group basis
- Experience of promoting health to children and young people with SEND
- Caseload management responsibilities
- Experience of mentoring/coaching colleagues
