Job summary

We are looking for an Advanced Nurse Practitioner to take on a new fixed term role to support the organisation.

You will be based at Waterlooville health Centre and work with a team of staff who are in clinical and non clinical roles. You will be expected to assess patients presenting with acute illness or for review of chronic disease conditions.

Main duties of the job

1.The practitioner will be autonomous and accountable for their professional actions and decision making.

2. Undertake clinical practice at an advanced level of clinical practice with expert knowledge and clinical skills evidenced at Masters level or equivalent as per the multi professional framework for advanced clinical practice to deliver holistic care to people accessing primary health care services.

3. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs and/or Long-Term Conditions (LTCs).

4. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

5. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

6. Refers patients as appropriate to other members of the multidisciplinary team, secondary care, other medical specialists for assessment, and to other statutory and voluntary organisations.

7. Refers to other members of the extended primary health care team as appropriate e.g. chiropodist, dietician, specialist nurses.

8. To actively implement safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escalate concerns.

JOB PURPOSE

1. Assess patients presenting with acute illness or for review of chronic disease conditions.

2. Skills and ability to consult and triage: Face to Face, by Telephone, and by Video Consultation.

3. To utilize practice IT (Emis/Adastra) for consultation and all other aspects of clinical care, including recording clear competent consultation notes and care plans.

4. Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

5. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

6. Assess diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

7. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

8. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

9. Fully participate in the clinical administration within the alliance.

10. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.

11. Work with patients in order to support compliance with and adherence to prescribed treatments.

2.1. Excellent communications skills with capability to communicate highly sensitive condition related information to patients, relatives and carers.

2.2. Provides and receives highly complex, sensitive or contentious information and frequently deals with situations which are hostile or emotive.

2.3. Uses developed communication, negotiation, conflict management skills which require empathy, reassurance and persuasive skills where there can be significant barriers to acceptance which need to be overcome.

2.4. Regularly communicates service-related information to GPs, operational managers and staff, Nursing and Administrative staff, patients, relatives and carers.

2.5. Makes operational judgements, manages conflicting views and reconciles inter and intra professional differences of opinion.

2.6. Builds and communicates therapeutic working relationships with all staff and external colleagues for the benefit of patient care and facilitates good working relationships.

2.7. Contribute to alliance/primary care targets both local and national within the disease management agenda e.g. Quality and Outcomes Framework (QOF), prescribing incentive scheme .

2.8. Encourage and motivate others to be forward thinking in ideas that address the health needs of the population that will be innovative whilst managing and developing new ways of working.

3.1. Develops and manages highly specialised programs of care and care packages and provides highly specialised advice concerning care normally beyond the scope of normal nursing practice e.g. establishes differential diagnoses, management/treatment plans and makes referrals as appropriate. Makes fast track referrals in line with local policy.

3.2. Initiates and carries out programs of health screening and ensures other team members are aware of health agendas and screening programs.
