Job summary

This is an exciting opportunity to join our In-Patient Unit, working as a Nursing Associate supporting the ward in all aspects of clinical care.

The successful applicant will have excellent communication and organisation skills as well as the ability to work as part of a multi-disciplinary team.

We are a 14 bedded hospice, serving the communities of Merton and Sutton by supporting those with life limiting conditions to live their lives as fully as possible. Caring for people at the end of their lives can be challenging, but it can also be immensely rewarding.

Job description

Job responsibilities

· To assess, plan and deliver care under the leadership of an RN to patients requiring palliative and/or end of life care.

· To offer timely and individual and holistic direct care based on the patient’s needs.

· To develop and evaluate patient care plans.

· To undertake risk assessments and re-evaluate prescribed care.

· To monitor patients for symptoms and provide non-pharmacological relief where possible, administering medication via all conventional methods and referring for senior appropriate.

· To be a second recorder for controlled drugs.

· To undertake various nursing skills (following being assessed as competent) as required such.

· To communicate fully and effectively with patients, their families and professional colleagues to ensure a safe and needs led care pathway.

· To work as a member of the multidisciplinary team to ensure a co-ordinated and defined management plan/plan of care for each patient.

· To maintain accurate healthcare records, written and computer based, liaising with community colleagues and in line with St Raphael's Hospice (SRH) organisational policy and professional requirements.

· To support and work with the IPU Sisters and Staff Nurses to develop and maintain effective communication links with members of the multi-disciplinary team

· To administer prescribed and medication in accordance with SRH guidance and policy

· To report accidents, incidents and complaints in area of responsibility as per procedure, participating in investigations as required

· To support and train junior staff within a defined remit

· To work to the processes for role boundaries and patient confidentiality

· To maintain personal and professional development

· To act within the Professional Bodies Code of Practice and NMC Code and Standards for Nursing Associates

Person Specification

Qualifications

Essential

  • Evidence of functional skills in Maths and English - GCSE grade A to C or equivalent.
  • Successful completion of the Nursing Associate Standard which includes attainment of a regulated leave 5 qualification such as a Foundation Degree or Level 5 Diploma of Higher education delivered by the NMC to provide preregistration nursing education.
  • Hold a valid NMC registration on the Nursing Associate register.

Desirable

  • Evidence of recent relevant CPD

  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
  • Understand and acts in line with NMC professional standards for practice contained within The Code
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
  • Understand of the importance of following procedures and treatment plans
  • Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
  • Insight into how strengths and development needs, seeking advice where appropriate
  • Experience of providing and receiving complex, sensitive information
  • Ability to take part in reflective practice and clinical supervision activities
  • Ability to organise and prioritise own delegated workload
  • Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team
  • Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
  • Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting
