Job responsibilities

Overall Purpose of the role

The provision of strategic and operational leadership and management of the Admission and Referrals services and Wandsworth End of Life Care Coordination services (WEoLCCS). The post holder will lead on ensuring these services meet the needs of the local population, are accessible and have clear patient pathways and processes. The role will include:

• Providing visible and effective leadership and management to all directly and indirectly managed staff which will achieve purpose and Trinitys vision and values.
• Develop and maintain an organisational culture which is inclusive and empowering, promotes trust, respect and responsibility and delivers positive and sustainable change.
• Develop and maintain organisational systems and processes which support the delivery of high-quality integrated patient care, team working and communication.
• Actively contribute to the Patient Services management team and hospice Senior Leadership Team, taking individual and corporate responsibility for effectiveness and development.
• To establish and maintain relationships with external stakeholders and represent the organisation in a professional and collaborative manner.

• Ensure IPU staff and volunteers are fully informed of issues, developments and updates from management meetings
• Build strong relationships with peers/colleagues/teams where there may be dissenting views
• Work collaboratively and creatively with team members to engage with service developments to ensure staff understand the wider context of care delivery such as the hospice strategy
• Review existing pathways and patient service processes and implement agreed changes
• Work with other Heads of Service to ensure patients/carers experience seamless care and support
• Communicate effectively at all levels within the organisation
• Demonstrate a sound awareness of the principles of FREDIE ((Fairness Respect Equality Diversity Inclusion and Engagement) and promote this to IPU staff and volunteers.
• Develop and maintain effective local, national and international networks and partnerships in order to promote and enhance the work of Trinity Hospice and our vision, values and strategic objectives

• Provide expert advice in relation to the provision of expert nursing in palliative and end of life care.
• Demonstrate breadth of experience as a senior practitioner in a healthcare environment
• Hold a relevant professional qualification and must be registered with the NMC.

Analytical skills

• Benchmark performance against national and local standards and highlight areas for improvement.
• Analyse, interpret and present data to highlight issues, risks and support decision-making

Planning and Organisation skills

• Ensure processes and procedures are in place to ensure compliance with policy, procedure and external guidance
• Horizon scan and be responsible for highlighting trends and issues that may influence practice
• Devise a robust strategy to provide high quality standard of service for all eligible patients

Physical skills

• Keyboard skills for regular use of computer systems including competent use of ms office Excel, Word, Outlook and PowerPoint
• Ability to travel to external meetings as required

Responsibility - patient/client care

• Provide Leadership in the triage and management of referrals and admissions to all patient services.
• To manage the activity of both services (A&R and WEoLCCS) and staff by the effective planning, prioritisation, and allocation of all appropriate resources.
• Manage and maintain robust systems for identifying, investigating, and managing any areas of concern in the delivery of both services, ensuring that remedial plans are developed to ensure safety of patients.
• Ensure statutory duty of candour, transparency and openness is embedded in processes related to patient and client care
• Ensure that those using our services have an opportunity to feedback about their experiences

Responsibility policy and service

• Ensure systems are in place that support the provision of high-quality care with effective monitoring and reporting that evidences quality of delivery
• Ensure provision of effective and timely expert, high quality support and guidance to operational services
• Develop and implement, or maintain and review, policies, programmes, and processes relating to both services, ensuring the hospice achieves its vision, values and strategy.

Responsibility finance & physical

• Accountable for the effective management of the budgets within both services
• Approval of fast-track CHC applications and overseeing and coordinating complex care packages.
• Responsible for physical assets and ensuring there is a robust system in place to monitor and control spending.
• Understands budget management and highlights risks/concerns at earliest opportunities

Responsibility staff/HR/leadership/training

• Proactively participate in all managerial activities and meetings as required
• Role model the values and behaviours expected of a manager in Royal Trinity Hospice, and enable others to do likewise by challenging others when necessary
• Responsible for overall staff management within both services; ensure that all A&R and WEoLCCS staff are recruited, inducted, managed and supported in line with Trinitys policies and procedures and to participate in those processes where appropriate
• Implement appropriate performance monitoring and management processes within the team to ensure that all aspects of performance and delivery are assessed and recognised with remedial action taken in a timely manner according to policy where agreed performance is not achieved.
• Motivate and enthuse staff at all levels to engage with/participate in clinical and corporate agendas research programmes and to produce publications
• In collaboration with Heads of Services develop professional development programmes for all nursing staff; to ensure those programmes are consistently and equitably implemented
• Responsible for ensuring all direct reports attend relevant mandatory training, have a personal development plan and receive appropriate supervision and appraisal.
• In conjunction with the Director of Patient Services to agree staffing levels for both services in accordance with patient needs and available financial resources
• To participate in the development and implementation of staff wellbeing and engagement programmes

Responsibility information resources

• Maintain personal identifiable and sensitive data according to information governance guidance and policy
• Ensure all documentation adheres to IG policy and process
• Maintain Asset Information Register for both services.
• Ensure all case notes are written contemporaneously.
• To participate in the development and implementation of risk management and business continuity planning for both services

Responsibility research & development

• Develop and implement systems to ensure that quality information is properly managed and that best practice is shared across all teams as appropriate
• Ensure data is provided to support the Senior Leadership Team in decision-making related to service development, quality monitoring and key performance indicators including weekly, monthly, quarterly and annual activity data and bespoke reports
• Ensure clinical data is provided for accreditation, commissioning and contracting requirements, and for oversight by the Board and Patient Services Committee
• Oversee infection prevention and control monitoring, evaluation, control, recording and action which ensures the Hospice is able to demonstrate compliance with all relevant legal/compliance requirements.

Freedom to act

• Capable of managing own workload without supervision and make senior management level decisions in accordance with organisational policy and practice

Person Specification

Qualifications

Essential

• Professional qualification and associated membership of a professional. Registered with the NMC

Desirable

• Masters degree in relevant subject

Experience

Essential

• Significant experience of nursing and service management within a health care sector setting including recruitment and retention.
• Experience of providing nursing leadership at a senior level and knowledge of relevant professional nursing issues.
• Significant experience of providing direct patient services.
• Experience of developing and maintaining effective inter-disciplinary team working structures and systems.
• Experience of managing challenging scenarios
