Job summary

As a member of the wider LPC nursing team and supporting the Blackburn with Darwen (BwD) PCNs, you will be responsible for delivering high-quality care at scale to the local population. Working autonomously and as part of a supportive clinical team, the postholder will provide a wide range of services including chronic disease management, wound care, immunisations, cervical screening, and health promotion. The role involves contributing to the delivery of population-level preventative care, supporting quality improvement initiatives, and mentoring junior staff such as HCAs.

You will support the BwD wider team in ensuring governance and safe care for our patients.

Main duties of the job

Primary Duties and Areas of Responsibility

  • General treatment duties including (but not limited to):
  • Wound and ulcer management
  • Administration of injectable medicines for patients
  • Contraception management
  • Prostate cancer management
  • Children and adult immunisations
  • Seasonal vaccinations
  • Well woman clinics
  • Participation in infection control audits and any other assurance protocols
  • Management of patients with long term conditions
  • Cytology consultations
  • Management of COPD, hypertension, asthma, diabetes and dementia

Management

  • Evaluate services to promote quality improvement
  • Manage available resources effectively in line with local guidance
  • Support the PCN teams as requested
  • Supervision of HCAs
