Job summary

The Digital and Transformation Lead will develop and implement a strategic plan to build a robust digital infrastructure for Blackburn with Darwen Primary Care Networks (PCNs).

The post holder will work with Clinical Directors and the leadership within the PCNs to create a vision - in alignment with ICS strategy - to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, expertise and leadership to deliver changes in a collaborative way.

The post holder will plan, coordinate and directly support delivery of transformation activity in general practice within a PCN, with a view to improving patient access, quality and continuity of care, staff and patient satisfaction, and the efficiency and sustainability of general practice services.

This is a site-based role with limited opportunities to work from home, to be discussed on request.

Salary is up to Band 8a Equivalent - dependent upon experience.

Main duties of the job

The postholder will develop a strategic digital plan for BwD practices and PCNs. This plan must:

o Be underpinned by a quality improvement methodology

o drive integrated working

o drive efficiency and reduce duplication

o reduce variation between practices

o be underpinned with easy-to-understand data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experience

o build staff skills and capabilities

o enhance patient access whilst protecting our workforce

o take into consideration the digital maturity of the PCN

o take into consideration the population's health needs and health inequalities within our community

o improve digital accessibility for our patients

The post holder will work closely with staff working in practice to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.

Working with PCN leadership, the post holder will help to develop strategy related to transformation and improvement, aligning this with larger scale strategy and initiatives, particularly at ICS level; they will contribute to collaborative problem solving with other system partners, patients and communities; focussing their time on the planning, implementation and evaluation of practical improvements that make a difference to patients and staff in the PCN.

To develop and maintain an in-depth understanding of the PCN digital infrastructure and business intelligence tools

To procure and manage digital tools for the PCN which is aligned to a framework of fundamental principles

To proactively project manage all digital projects and also evaluate the effectiveness of theintervention

Work alongside practices to understand current challenges to effective service delivery and identify solutions, including existing best practice within the PCN

Drive the planning and prioritisation of changes that will improve delivery of care and improve patient and staff experience in the practices within the PCN.

Asses the PCNs transformation maturity and help the ICS understand local support needs,for example, facilitated by using relevant frameworks

To ensure all initiatives are supported by a clinical champion and practice management

To lead the development and implementation of digital transformation bids

Provide hands-on capacity and support to practices in implementing and sustaining agreed changes

To ensure all staff receive training on the digital tools within the PCN

To ensure the induction of all new members of staff receive training and resources on tools available

To ensure all initiatives are fully scoped and underpinned by a quality improvement methodology

To remain up to date with the national and local PCN guidance.

To work in partnership with the PCN leadership and management team

To identify and develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice

Develop relationships with peers across other PCNs to share learning and expertise at place or system level.

Develop relationships with the ICS to discuss and agree local transformation plans and activity, and to support alignment with broader strategy, including the ICSs digital strategy.

Working with relevant colleagues in the ICS, the post holder will also develop relationships with local delivery partners (such as IT service providers and other NHS organisations) to support effective delivery of transformation.

Work with the ICS to request specialist IT and estates expertise required to deliver transformation plans, in line with the ICSs delegated responsibilities for GP IT and GP estates. The post holder will also work with the ICS to bring in other specialist support as required e.g. in analytics, evaluation, organisational development etc.

To deliver technical information in a non-technical and engaging way to a range of stakeholders

Recognise the roles of other colleagues within the organisation and their role

Foster and maintain strong links with all services across locality

Explore the potential for collaborative working and take opportunities to initiate and sustain such relationships

Liaise with other stakeholders as needed.

Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made

Qualifications

Essential

  • Masters degree (or equivalent professional experience)
  • Professional qualification or experience in IM&T to demonstrate subject matter expertise
  • Project management qualification, such as PRINCE or equivalent experience

Desirable

  • Managing Successful Programmes (MSP) qualification

Experience

Essential

  • At least five years experience in Digital / IM&T project management or
  • transformation experience in a Health or Social Care Environment
  • Experience of working in general practice with a good working knowledge of general practice systems (including IT, digital and BI tools), processes, and procedures; or experience of successfully delivering change and improvement programmes in a patient (or customer) facing environment
  • Significant experience of using programme management and improvement techniques to drive change forward, including
  • translating strategic objectives into tangible plans for delivery and following these through to benefits realisation
  • Experience in project delivery through the entire project lifecycle from initiation to delivery
