We have exciting opportunities for caring and committed professionals to join us and provide a truly holistic approach to supporting patients within our In-Patient Unit.

You will be a valuable member of the In-Patient Unit team and wider multidisciplinary team, assisting with the delivery of high quality care. You will carry out a wide range of care duties to meet the holistic needs of the patient and those important to them.

This is a Bank role on an 'as and when required basis' and could be combined with other work commitments.

This post requires and Enhanced Disclosure and Barring Service check.

Main duties of the job

About you

You will put patient care at the heart of the work you do and be able to communicate clearly and effectively to patients, those important to them and colleagues.

You will have experience of working in a team and interacting with members of the public, you will be required to work independently under the direction of our team of registered nurses, in a supportive and collaborative environment.

Experience of working with adults in a care setting and a Care Certificate or NVQ level 2 in care would be desirable but not essential.

Job Summary: Be an effective, valuable member of the In-Patient Unit team and wider multidisciplinary team to assist with the delivery of high quality care to patients and those important to them. Carry out a wide range of duties to meet the holistic needs of the patient and those important to them.

Contribute care that enables the the In-Patient Unit team to provide a service that is safe, effective, caring and responsive.

Main Duties and Responsibilities:

Clinical

Demonstrate awareness of own professional role and boundaries, acting in accordance with the code of conduct for healthcare assistants (2013).

Maintain accurate, contemporaneous nursing records and communicate effectively.

Analyse models of care and person-centred approaches and have influence on palliative and end of life care in the In-Patient Unit.

To deliver a high standard of holistic care to patients and those important to them, undertaking clinical skills as requested by the registered nurse.

Promote and uphold the privacy, dignity, rights, health and wellbeing of patients and those important to them at all times.

Work in a person-centred way, listening and taking into account the wishes of the patient and those important to them.

Analyse the range of causes influencing symptoms and symptom experience.

Demonstrate knowledge of major cultural and religious customs which relate to the delivery of palliative and end of life care.

Communicate effectively with a range of people in a manner that is appropriate to them.

Work effectively and collaboratively with colleagues to ensure a high standard of safe and compassionate care is provided, sharing information appropriately whilst taking into account confidentiality. In partnership with others, review assessments to take account of changing needs, priorities and wishes.

Demonstrate an understanding of Advance Care Planning, know what the Advance Care Plan contains and how this will impact the persons care.

Contribute to audits, research and surveys when requested as part of a team.

Education and Learning.

Recognise own knowledge and skills and limitations, seeking support where required.

Demonstrate support and mentorship for colleagues, new starters and students when required.

Take responsibility as a link worker for a specialist subject area and a. Build relationships with specialist colleagues b. Cascade learning/competency across the team
