Job summary

As we continue to evolve our services to meet the needs of our community, we are looking for a Senior Staff Nurse to join our Hospice at Home team.

The role will provide a responsive, flexible and high-quality community specialist palliative care service to patients and those important to them.

You will;

  • Provide specialist nursing care to patients in the last weeks/days of life.
  • Complete assessments of patients referred to the Hospice at Home service at any stage in their palliative diagnosis.
  • Transfer patients care to other hospice services as appropriate when nursing interventions are no longer required.
  • Work independently during weekends and as part of a 7-day rota, including bank holidays.
  • Lead and coordinate care planning, ensuring safe, effective, and personalised care.
  • Delegate appropriately and support the work of Senior Healthcare Assistants (SHCAs).
  • Collaborate closely with the wider multi-disciplinary team (MDT).

Main duties of the job

We are seeking a highly skilled and experienced nurse who is:

  • Ideally experienced in community nursing and/or palliative care.
  • Confident working independently, including lone working at weekends.
  • Committed to developing professionally within a supportive and values-led environment.
  • Skilled at delegating tasks and coordinating care across a team.
  • A strong communicator with the ability to work collaboratively with an MDT.
  • Driving licence and access to a vehicle are essential for work purposes.
