The Big Picture

Bury GP Federation has an exciting opportunity for an Advanced Practitioner with a nursing background (ANP) to work with the Primary Care Networks (PCN); Horizon, Whitefield & Prestwich. The role will be leading & supporting a team of experienced paramedics & care co-ordinators to deliver a clinical service. The service undertakes acute same day visits as well as some pro-active & planned consultations.

The role will cover all four pillars of advanced practice, including the leadership & education of a growing team, management of services, autonomous clinical practice, & research in the form of audits. Undertaking quality improvement projects & leading the evolution of the service will be a key element of the role.

We are looking for an ANP with at least two years non-medical prescribing practice who will be able to supervise trainee NMP's and trainee ACP's as a practice educator. The successful candidate will also support the development of trainee Nurse Associates & leadership of Nurse Associates in the service as well as across the wider locality.

Main duties of the job

Work within the community setting, as an autonomous ANP providing holistic care to patients. This will include assessing, managing, and treating patients in their home. This is a new service within Bury locality and there is an opportunity for you to be an integral part of its evolution. The role will involve working alongside local practices, building strong working relationships, as well as to provide an acute visiting service primarily staffed by ACP's & paramedics. Part of the role will involve supervising & mentoring clinicians working in the service.

Clinical practice will include primarily home visits. There will also be the opportunity to develop into other services in the future including

  • Scheduled primary care consultations in the community
  • Same day primary care consultations in practices
  • Scheduled primary care consultations in practices

Leadership and management may include:

  • Representing Bury at locality & regional forum
  • Line management of clinical & support staff including appraisals
  • Governance of others clinical practice
  • Determination of & delivery of service improvements
Education of self and others may include:

  • Any necessary role-specific training & education
  • Leading clinical education events
  • Acting as Prescribing Supervisor for NMP students
  • Supporting trainee ACP's
  • Supporting clinicians appraisals action plans
Research may include:

  • Audit of team & service level outcomes
  • Initiation of primary care research projects
  • Delivery of research projects
