We are excited to
share an opportunity for Registered Nurses to work in City and Hackney and
qualify as a General Practice Nurse.
You will be employed
by a local practice where you will be based and gain experience of working in a clinical
setting, whilst undertaking a Postgraduate Diploma in General Practice Nursing.Please note that the location for each trainee post will be a GP practice in City & Hackney (not the GP Confederation).
Every trainee will
have an experienced GPN as their mentor/supervisor during their placement,
alongside receiving support from our VTS Scheme Co-ordinator.
The GPN VTS scheme
offers a one-year fixed term contract based on mid-point Band 5 £35,016.00 inclusive
of HCAS.

Main duties of the job
The
GPN Vocational Training Scheme is crucial to our aim in City & Hackney of developing
opportunities for nurses new to primary care. You will have access to
a structured programme of education, working with our academic partners, our practices and other
providers. The scheme provides the opportunity for postholders to develop both the academic
underpinning and clinical competencies required in General Practice Nursing.
The
GPN trainees will each be employed by a host practice who meet a set of agreed
criteria. The trainees will receive support in the GP practice where they are
working from a Practice Nurse mentor who will be an experienced GPN working at
the practice. The trainees will be supernumerary and will also undergo a 12-month
level 6 Competency-based training programme at university, leading to a
recognised practice nursing qualification.
Job
Purpose:
To
develop the knowledge, clinical skills and attitude to provide high quality
care and services based on the needs of the practice population. Supported by
senior nurses within the practice, you will deliver care within the boundaries
of your role, focusing upon supporting patients to be healthy, monitoring of
long-term conditions, health promotion and screening activities. You will work
collaboratively with the general practice team to meet the needs of patients,
supporting the development and implementation of policies and procedures.

and Hackney GP Confederation is a Community Interest Company with a membership
of all 39 GP practices in City and Hackney. The purpose of the Confederation is
to support our member practices to improve quality, resilience and stability in
primary care, and to develop innovative, cost-effective services that respond
to the needs of local people.
GP
Practices are managing a number of nursing vacancies and the turnover of staff,
in some practices, is increasing. There are also a number of current General Practice
Nurses planning to retire over the next five years. We are seeking to assist
practices and primary care networks to develop new services and approaches in
Primary Care, and attract the right nursing workforce into general practice in
our area.
This scheme provides a supportive and educational experience to enable qualified nurses to develop their skills in primary care nursing. The aim of the scheme is to provide each trainee with a really positive learning experience and to encourage them to want to continue working in general practice in City & Hackney once they have completed the scheme.

Clinical Responsibilities
1.
Support
patients to adopt health promotion strategies that encourage patients to live
healthily, and apply principles of self-care;
2.
Deliver
opportunistic health promotion using opportunities such as new-patient medicals
and NHS health checks;
3.
Implement
and participate in national vaccination and immunisation programmes for both
adults and children. Provide travel advice and appropriate vaccinations;
4.
To
manage patients with long term conditions, running specific clinic sessions and
maintaining robust recall systems;
5.
Assess
and care for patients presenting with uncomplicated wounds. Removal of sutures
and clips;
6.
Support
and manage health needs of women presenting for cervical cytology consultations and
undertake cervical cytology screening;
7.
Support
and advise patients requesting information relating to reproductive and sexual
health;
8.
Prioritise
health problems and intervene appropriately to assist the patient in complex,
urgent or emergency situations, with backup from senior members of the team;
9.
Assist
senior practitioners in providing minor-surgery sessions;
10.
Recognise,
assess, and refer patients presenting with mental health needs in line with
local policy and practitioner competence.
11.
Assist in
and perform ECGs, Urinalysis, Peak Flow Readings, Venepuncture Blood Pressure
measurements fitting of and recording 24-hour BP readings and other specific
tests as appropriate for the individual Practice requirements
Communication
1. Communicate effectively with patients and
carers, recognising the need for alternative methods of communication to
overcome different levels of understanding, cultural background and preferred
ways of communicating;
2. Utilise communication skills to support
patients to adhere to prescribed treatment regimes;
3. Anticipate barriers to communication and
take action to improve communication issues;
4. Act as an advocate when representing the
patients and colleagues viewpoints to others.
Administration
1. To maintain accurate patient records and
enter onto the computer using agreed Snomed codes;
2. Ensure accurate completion of all
documentation associated with patient care;
3. To collect data and participate in regular
practice audit and research projects;
4. To contribute to any practice development
plans.
General
1.
To
maintain own annual registration with the NMC and professional indemnity;
and adhere to The Code:
Professional standards of practice and behaviour for nurses, midwives and nursing
associates;
2.
To
maintain a safe and therapeutic environment for all Primary Health Care Team staff,
patients, carers and colleagues from allied organisations;
3.
Ensure
all statutory regulations (e.g. Health & Safety, COSHH, etc.) are
implemented and adhered to;

times by attire and attitude to patients and staff both in and out of the
Practice;
