Job summary
Responsible for working at
an advanced practice level to provide a co-ordinated and comprehensive
specialist palliative care service, to patients and their loved ones facing a
terminal illness within the
hospice catchment area.
Main duties of the job
Working within a skill mixed team, you will be responsible for a caseload of community palliative patients. This will include supporting appropriate identification and referral of patients, conducting comprehensive holistic assessments, diagnosis, recommendation of evidence-based symptom management plans and enable advance care planning within the community and out-patient setting.
The post-holder will be an expert role model, providing education and support regarding palliative care issues to other health and social care professionals, in conjunction with senior clinical management, stimulate and direct the development of clinical practice across the local community.

Essential
- Registered Nurse with the Nursing and Midwifery Council (NMC) with a Level 1 Nursing Qualification with a minimum of 5 years post registration experience
- Or
- Registered Paramedic with the Health and Care Professionals Council (HCPC)
- Successfully completed an IHCD Paramedic Training Programme or a recognised Higher Education Paramedic qualification.
