Job summary

An exciting opportunity has arisen for 2 All-Age Immunisation and Population Screening Coordinators, who must be available to work on a full-time basis at FedNet with flexibility when required to work across sites with PCNs. Practices across Waltham Forest have come to work collaboratively as Primary Care Networks (PCNs), pooling their resources and workforce to provide improved and integrated services for their patients. PCNs typically consist of 30,000-50,000 patients and the All-Age Immunisation and Population Screening Coordinator will provide hands-on support to general practices to improve immunisation and population health screening uptake.

Main duties of the job

Duties will include but are not limited to:

  • Working with practices on their call/recall initiatives and ensuring improved processes are embedded
  • Provide training and guidance on GP clinical systems, including coding
  • Work with system leaders on ensuring best practice guidance is followed and implemented in practices to improve uptake and reduce variation
  • Liaise with key stakeholders such as LA, local communities to reduce local variation and local inequalities
  • Work with practices to take on a whole systems approach to immunisations and population screening
  • Review, analyse and present complex information
  • Provide feedback to practices and FedNet about issues and concerns around immunisation and population health screening;
  • Sourcing guidance and training
  • Attend appropriate local strategic meetings to inform and support the implementation of immunisation and population screening strategies
  • Provide monthly updates to CCG/ICS commissioners and monthly reports including outcomes/KPIs

NHS England London Public Health Department has responsibility for the commissioning of NHS Section 7a population screening and immunisation programmes.

  • The role of the immunisation and population screening coordinator is to contribute to the realisation of the London Immunisation and London Screening Strategy and the improvement in uptake to meet the national targets for the vaccination and population health screening programmes in London
  • The post holder will contribute to developing the system’s immunisation and population screening services at a local level aiming to eradicate the vaccine-preventable disease from London
  • The post holder will work as a valued member of FedNet and the primary care teams supporting the introduction of new immunisation programmes, screening programmes and the extensions to existing ones, in line with national requirements.
  • The role supports the delivery of best practices, addressing inequalities and supporting value for money in service delivery.

Key Job specifics and responsibilities

The post holder will report to the Medical Director and be accountable to the FedNet board. The post holder will support the local commissioning of nationally defined immunisation and population screening programmes in Waltham Forest. The post holder will work to deliver the NHS EI London Immunisation and Population Screening Strategies in synergy with local requirements, primary care colleagues and local stakeholders. The post holder will be expected to travel across Waltham Forest as part of the day-to-day role and will work collaboratively with a wide range of external stakeholders.

Key Activities for Immunisation and Population Screening Coordinator:

  • Identify and work with local practices on data recording for vaccinations and population screening programmes.
  • Work on-call/recall with local practices and ensure processes are embedded.
  • Develop practice reports for monitoring uptake rates of vaccination services and providing guidance on how to improve these.
  • Improve competencies of practice staff on vaccination and screening services and increase confidence levels.
  • To attend professional network meetings in the area
  • Develop and Implement healthcare professional initiatives that are required for the local area.
  • Share good practices with extended stakeholders
  • Provide monthly update reports to CCG/ICS Commissioners and internally to the Contracts and Quality Lead.

Improving quality and outcomes

  • To project manage clinical and assurance audits
  • To signpost to specialist public health advice where required.
  • To provide coordination of and participate in relevant meetings, reporting attendance
  • To administratively support multidisciplinary working relationships across public health and other care providers.
  • To be responsible for a high standard of work supporting the delivery of projects on time, to quality standards and in a cost-effective manner

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in a specialist area
  • Further training or significant experience in project management, clinical systems, financial/contract management or supporting change management processes.

Desirable

  • Educated to Post-graduate degree level in a relevant subject e.g. Public Health, Healthcare Commissioning or another relevant subject
